Shared Care Scheme
CircleBath Hospital
Mr Richard Antcliff, Mr Roger Baer, Mr Jonathan Boulton, Mr Jonathan Luck, Miss Fiona Cuthbertson and Mrs Sally Webber, Consultant Ophthalmologists, carry out cataract surgery at CircleBath.
CircleBath are working with Optometrists offering a shared care scheme to patients who have their surgery carried out at CircleBath.
The scheme works as follows:
The Optometrist completes and faxes a copy of the Private Cataract Surgery form to the number shown on the bottom of the form.
The patient is contacted by CircleBath and a consultation arranged.
The patient, as long as he/she is suitable, has cataract surgery carried out at the hospital.
The patient returns to CircleBath to see the consultant for the appropriate check up(s).
If all is well, and there is no further need for hospital supervision, the patient returns to the original referring Optometrist one month after surgery for the final follow up refraction.
The Optometrist completes the Shared Care Referral form and faxes it back to CircleBath.
The Optometrist receives a £38.00 for the work involved and the follow up information will be used for audit purposes.

Laser Eye Surgery
Shared Care Scheme
CircleBath Hospital
Mr Jonathan Luck, Consultant Ophthalmologist, is now carrying out Laser Eye Surgery at CircleBath.
CircleBath are working with Optometrists offering a shared care scheme to patients who have their surgery carried out at CircleBath.
The scheme works as follows:
The Optometrist completes and faxes a copy of the Laser Vision Correction/Lens Implant Surgery Shared Care Referral form to CircleBath as shown on the bottom of the form.
The patient is contacted by CircleBath and a consultation arranged.
The patient, as long as he/she is suitable, has either Laser Vision Correction or Lens Implant Surgery carried out in the specialist laser operating theatre at the hospital.
The patient returns to CircleBath to see Mr Luck for number of check ups.
If all is well, and there is no further need for hospital supervision, the patient returns to the original referring Optometrist three months after surgery for the final follow up refraction.
The Optometrist completes the Shared Care Referral form and faxes it back to CircleBath.
The Optometrist receives a £75.00 for the work involved and the follow up information will be used for audit purposes.
The price for Laser Eye Surgery at CircleBath is £1800 per eye. (as at Oct ’11).
