FY 2014 PUBLIC HEALTH MASTER AGREEMENT ANNEX

Program Description and Reporting Requirements

PROGRAM NAME: Adolescent Health and Youth Development

PROGRAM CODE: 306

FUNDING SOURCE: TANF

PURPOSE: The Adolescent Health and Youth Development (AHYD) Program provides evidence-based programs and strategies to prevent pregnancy, HIV/STIs and chronic disease among Georgia’s adolescents. Funds will be allocated to the Lead Agency in the following Health Districts: Cobb/Douglas, Fulton, Clayton, Gwinnett, DeKalb, Macon, Richmond, Dougherty and Coastal.

FUNDING REQUIREMENTS:

-  Restrictions: Secure/maintain a full-time District Youth Development Coordinator and/or AHYD Program Staff

-  Funds shall not be used to supplant local funds.

-  Program funds not utilized by the end of the fiscal year shall be returned to the state office.

-  Indirect costs shall not be charged to this program unless a cost allocation plan has been approved by the Division of Financial Services.

-  Allowable costs/expenses shall include travel, personnel, personnel background checks, supplies and educational materials/pamphlets.

-  Funds cannot be used to purchase equipment without approval from the State AHYD Program.

-  Funds cannot be used to purchase furniture or to construct, purchase or renovate a building/space.

-  No more than 2.5% of funds can be used to purchase health education and outreach materials. All items must be approved by the State AHYD Program prior to purchase.

-  Funds may be used to purchase food for youth program participants. No more than 1.5% of funds can be used to purchase food for adults when attending an AHYD event.

Deliverables:

Primary

¾  Implement the Personal Responsibility Education Program (PREP) as outlined in the approved work plan, Standards for graduating youth can be found in the “GA-PREP Operations Manual.” Attend and participate in required training as identified by DPH/DFCS.

¾  Plan and implement a minimum of three (3) public awareness events around adolescent health-related issues/topics.

¾  Provide a minimum of three (3) training opportunities to youth-serving professionals, parents, community members or youth.

Secondary

¾  Establish or maintain membership on a stand-alone Adolescent Health and Youth Development Coalition.

¾  Explore and secure non-TANF sources of funding to sustain a comprehensive and coordinated system of district-wide supports and services.

PERFORMANCE MEASURES: (## below to be agreed upon in individual district work plans)

Objective 1:

Between July 1, 2013 - September 30, 2013 complete PREP implementation with a total ## of at-risk youth, including ## youth in foster care. (PY-2)

Between October 1, 2013 – June 30, 2014 complete PREP implementation with a total ## of at-risk youth, including ## youth in foster care. (PY-3)

Objective 2:

By June 30, 2014 plan and implement ## public awareness events around adolescent health-related issues/topics.

ALLOCATION METHOD:

Funds will be allocated to the following Boards of Health for the benefit of the counties in the referenced Health Districts:

Cobb County Board of Health – District 3-1

Fulton County Board of Commissioners – District 3-2

Clayton County Board of Health – District 3-3

Gwinnett County Board of Health – District 3-4

DeKalb County Board of Health – District 3-5

Bibb County Board of Health – District 5-2

Richmond County Board of Health – District 6

Dougherty County Board of Health – District 8-2

Chatham County Board of Health – District 9-1.

Upon successful completion of the approved work plan, continued demonstration as a key priority area for DPH and/or fund availability these Districts will receive this continuation of funding.

REFERENCES:

“Georgia PREP Operations Manual” is provided to each Youth Development Coordinator.

REPORTING REQUIREMENTS:

The AHYD Program Consultant will provide technical assistance and training to the District Youth Development Coordinators and monitor performance in achieving programmatic deliverables. Monitoring will occur via site visits, conference calls and review of reports.

Submit quarterly reports using the web-based iProgress Check System no later than the 5th day after the end of each three month period during the term of this agreement. In the event the online reporting system is unavailable, Districts must submit an electronic report to: . Reports are due by: October 7, January 6, April 7, and June 30.

Any change in staff shall be submitted to the State AHYD Program within 10 days of the change.
