CHINA TOURIST ENTRY VISA SERVICE INSTRUCTIONS — U.S.

Please send us the following documents, along with a check for your fees:

1.  VALID PASSPORT — You must send your actual passport. The passport must have at least 12 months of validity remaining before expiration and have at least one completely blank page for the visa stamp.

2.  VISA APPLICATION FORM — Download the application form at http://www.china-embassy.org/eng/visas/faq/ and fill it out. After you have completed the form, print it and sign it. Your form must be typed or done on the computer. The Chinese Embassy will NOT accept handwritten forms.

3.  PHOTOGRAPH — You must provide one 2”x2” passport-type photo. Homemade digital photographs are not acceptable.

4.  PHOTOCOPY OF YOUR DRIVER’S LICENSE OR IDENTIFICATION CARD.

5.  SUPPORTING DOCUMENTS — If you apply before the end of 2015, Spring International will provide supporting documents (e.g., itinerary, airline reservation).

6.  PREPAID FEDEX ENVELOPE OR INCLUDE MAILING FEE (SEE BELOW) — You must include a prepaid FedEx envelope or pay one of the mailing fees listed below.
