I, the parent or legal guardian of the above named applicant, do hereby give my consent for his/her participation in the above named program sponsored by the Erwin Parks and Recreation Department so long as he/she abides by the rules and regulations as established for the program. I agree to assume full responsibility for any risks, accident or injury to the participant while he/she is engaged in the program or necessary transportation incidental to the program. I do hereby in consideration of his/her acceptance as a program participant, waive, release, absolve and agree to hold harmless the Town of Erwin, The Erwin Parks and Recreation Department, The Parks and Recreation Advisory Commission, their employees, volunteers, organizers, sponsors and supervisors from any claim arising from any accident or injury to the participant. This participant may not begin practice until this waiver is properly signed.Parent/Guardian signature also guarantees cost or return of uniform and/or equipment, if applicable

CODE OF CONDUCT

“Youth sports are for kids – not adults!”

Everyone shall conduct him/herself in a sportsmanlike manner at all times:

Cheer FOR your team, NOT AGAINST the opponent;

Praise both teams – they’re all kids

Spectators SHALL NOT act as a coach and/or official. Let the coaches coach and the officials officiate.

Verbal abuse WILL NOT be tolerated:

Profanity, vulgarity, obscenities, mockery, sexual harassment, racial slurs, threats, degrading, and/or demeaning comments

Physical force, physical threats, and/or obscene gestures WILL NOT be tolerated.

Everyone shall abstain from possession, consumption, and the influence of alcoholic beverages and/or drugs while at recreation facilities.

Please express any and all concerns of those not following these guidelines with host management and/or your own department.

Failure to follow these guidelines shall result in removal from the recreation facilities as well as possible suspension from all recreation activities until the suspension has been fulfilled.
