Cats RIDDE is a student run program at the University of Arizona that aims to provide a safe, free, and reliable designated driving service to promote the safety of fellow students, and improve the campus community. Cats RIDDE is not a limousine or taxi service.
(1) I am a University of Arizona student. I understand that Cats RIDDE reserves the right to terminate a ride at any time.
(2) I understand that Cats RIDDE is only available to accept phone calls on Saturday evenings-Sunday mornings from 10:00PM-2:00AM.
(3) I agree to show proof that I am a UA student before a ride to my local Tucson, Arizona place of residence will be provided.
(4) I will comply with the University's rules, standards and instructions for student behavior, including the Student Code of Conduct. I understand that I am not permitted to consume alcohol, use tobacco, possess/use illegal substances or weapons, or engage in sexual/illicit behavior while in a Cats RIDDE vehicle. I waive and release all claims against the University that are caused by my failure to comply with such rules, standards, and instructions.
(5) I understand that I will be reported to the police if there is an immediate threat of harm to Cats RIDDE volunteers or the University community. Violations of the UA Student Code of Conduct will also be reported to the UA Dean of Students Office.
(6) I understand that Cats RIDDE reserves the right to alert the appropriate University or law enforcement officials or medical personnel if I become ill while being transported. I agree to be responsible for the costs of any medical treatment and medical transportation.
(7) I will only be able to use this service for transportation to my local place of residence and not to any other location, or to a car.
(8) I will wear my seatbelt while inside any Cats RIDDE vehicle.
(9) The University of Arizona reserves the right to modify the schedule of Cats RIDDE or cancel the program at any time without notice to the users.
(10) Risks of Participation. I fully recognize that there are dangers and risks to which I may be exposed by participating in Cats RIDDE. I understand that the University of Arizona and its governing board, officers, employees, and agents (collectively the “University”) does not require me to participate in Cats RIDDE, but I want to do so, despite the possible dangers and risks and despite this Release. I therefore agree to assume and take on myself all of the risks and responsibilities in any way associated with Cats RIDDE.
(11) I agree that the University has the right to enforce the standards of conduct described above, in its sole judgment, and that it will impose sanctions, up to and including expulsion from the Cats RIDDE program or from the University, for violating these standards or for any behavior detrimental to or incompatible with the interests, harmony, and welfare of the University, the Cats RIDDE program, or the University community.
(12) Assumption of Risk, Covenant Not To Sue, and Release of Claims. Knowing the risks described above, and in consideration of being permitted to participate in the Cats RIDDE program, I agree, on behalf of my family, heirs, and personal representative(s), to assume all the risks and responsibilities surrounding my participation in Cats RIDDE. To the maximum extend permitted by law, I release, indemnify, and covenant not to sue the University from and against any present or future claim, loss or liability for injury to person or property which I may suffer, or for which I may be liable to any other person, during my participation in Cats RIDDE.
I have carefully read this Registration and Release Form before signing it. No representations, statements, or inducements, oral or written, apart from the foregoing written statement, have been made. This agreement shall be governed by the laws of the state of Arizona, which shall be the forum for any lawsuits filed under or incident to this agreement or to the Cats RIDDE program.
