Definitions
A person-centred approach respects the person and treats them as an individual, focusing on their needs, abilities and achievements. It takes into account the whole person’s needs.
A personal plan is an individual or group plan for service delivery, developed by service providers together with the person or people receiving support. The plan may include their family/whānau as appropriate. It may include a care plan or a rehabilitation plan.
An organisation’s policies and procedures are the policies and procedures of the employer and include ethical codes, standards and other organisational requirements.
Assessment note
For the purposes of this assessment, contribute to means contributing to the process under the guidance of a senior health professional.
Task 1: Assessment tools

Task 2: Contribute to personal planning
For this task, you need to show how you have contributed to personal plans for two different people with complex needs. For each person you must have contributed to the:
· assessment of their complex needs.
· planning and development of their personal plans according to the assessment.
· implementation of their personal plans.
· review of their personal plans.
· updating of their personal plans based on the review.
One of these two people you support must be Māori, Pasifika and/or from another culture.
Complete all the written parts of this task before asking your verifier to complete the verification checklist.

Verification
