Dear Parents/Guardians,
Villa Rica Middle School will be sponsoring a Spring Semi Formal dance on Friday, May 13th from 6:00 PM until 9:00 PM. “Early bird” admission to the dance will be $12.00 per student and a signed permission form will be required in order to attend. “Early bird” begins April 25th and runs through April 29th. From May 2nd to May 6th the price goes up to $15.00. All monies and permission forms are due no later than Friday, May 6th to Ms. Mayfield. No money or permission forms will be accepted after Friday, May 6th. This is strictly enforced. Ms. Mayfield will be available to collect money/forms from you starting on Monday, April 25th in the mornings and at lunch. Also, pictures will be available for purchasing (see pricing on the back). Make all necessary transportation arrangements prior to the dance. Also, if any student has a library fine or still owes money to another organization (sports/clubs/lunchroom), it must be paid before admission will be accepted. The earliest time students can check out and still be excused will be 2:30 p.m. on the day of the dance.
We are sponsoring the semi-formal for students with positive behavior and good attendance, therefore the students with the following behavior/attendance issues will not be allowed to attend:
* Students who have been in ISS twice since the beginning of February 2016 will not be allowed to attend.
*Students who have been in OSS at any time during the school year will not be allowed to attend.
*Students who have more than 10 days absent without surgery or chronic disease involved, may not attend. Doctor’s excuses only accepted.
*Parents are not allowed into the dance. Thank you for your cooperation.

DANCE ATTIRE:
This is a semi-formal dance. Students who show up in “street clothes” will not be allowed to enter. “Street clothes” includes jeans (no sagging pants), tennis shoes, tshirts, or any other type of clothing students would normally wear on a day to day basis.
Girls: Please dress age appropriate. No plunging necklines, showing too much cleavage, or too short dresses. Remember you will be dancing, so do a mirror check to make sure you will not expose anything. If you are unsure whether your dress is appropriate, you can take a picture of you in it and show Ms. Mayfield.
Boys: Dress khakis or dress slacks and a button down shirt with a tie.
