Tier 4 – Bariatric Surgery
Description / Surgical intervention for morbidly obese patients (including 2 years post-surgery follow up)

Criteria /
  • BMI > 40 or BMI >35 with significant co-morbidities
  • Aged between 18-60
  • Morbid/severe obesity has been present for at least 5 years
  • Have been receiving Tier 3 obesity management for at least 12 months (6 months for BMI >50) and have tried all appropriate and available non-surgical measures adequately but have not been able to maintain weight loss

Tier 3 –Specialist Multi-Disciplinary Team Weight Management
Description / This service offers full medical assessment, and a programme of structured dietary and exercise advice, as well as access to weight loss medication, low calorie liquid diets and assessment for suitability for weight loss surgery. A 6 or 12 month programme is available for patients.

Criteria /
  • Aged 18 or over
  • BMI >40 or BMI >30 plus co-morbidity

Referral / Referral letter to be sent to the Individual Funding Request panel (for activity monitoring purposes only) containing:
  • Patients current BMI
  • Details of any co-morbidities
  • Confirmation that morbid/severe obesity has been present for at least 5 years
The IFR Team will then forward the referral to the specified provider.

Tier 2 – Targeted Programmes
Description / The MyTime Health Trainers work with patients to improve diet, increase physical activity, reduce alcohol intake and give up smoking. A Health Trainer will see clients on a one-to-one basis for up to 6 visits within a 6 month period depending on individual needs, if additional help is required they can attend maintenance appointments for a weigh-in and further brief advice.
Locations / Health Trainers run regular clinics at GP surgeries and Medical Centres across Norfolk and attend many local events to promote wellbeing and good health to the community
Criteria /
  • Aged 16 or over
  • Contemplating or making lifestyle changes
  • Prepared to attend 1:1 sessions or group health trainer programme

Referral / Accepted via the Health Trainer Referral Form (available here) from any Health Professional or local organisation. The service also accepts self-referrals.
