Four (4) copies of this application form must be completed and returned to Sheri Amundson, Finance Staff Accountant, City of Arlington, 238 N Olympic Arlington WA 98223, along with four (4) copies of your official budget document. The completed application must be postmarked within ninety (90) days of legal adoption of the budget.Please note question 5. Please include a $50 handling fee for applications submitted to WFOA, after your entity has received their initial WFOA or GFOA Distinguished Budget Award.
