Referral Guide for Partners

VR referral guide:

Is this person a student in secondary education?:

If Yes: Students will need support of parent/guardian.

Students also need 1 community based work experience (paid or unpaid)

*YTP students who are engaged in activities can use this experience

Does this person have a personal agent or DD service coordinator?:

If Yes: Please contact the PA or service coordinator prior to meeting and notify them of

referral.

To bring completed to first appointment:

Completed Service Questionnaire (can be found online or obtained from VR office)

Current (Unexpired) Government Issued Photo ID

Documentation of Ability to Work in United States (Social security card, birth certificate, passport, Green Card, Tribal Documentation...etc.)

*VR Counselor cannot meet with a new client without appropriate identification documentation.

First appointments typically last 1-1.5 hours.

If individual is under 18 or under court appointed guardianship, the guardian must be present.

*Guardianship paperwork must be provided if applicable.
