The following information will assist you on your visit. Covered parking is available in the South Garage, located across from the Monroe Carell, Jr. Children’s Hospital at Vanderbilt. You can access the building on the first level of the garage or valet park for free at the front of the building by turning onto24th Avenueand then onto Pierce Avenue.
CHECKING IN:
Upon arrival to the clinic, please sign in at the service desk on the first floor and follow instructions to appropriate clinic. One of our Patient Service Coordinators will assist you in the process. Once at the specialty clinic for your visit, a service representative will check you in for your visit. Please have your Insurance card(s) and Co-payment ready at this time. This will help speed up the check in process. If you do not have your Insurance Cards or Co-payment you may be asked to reschedule for a later date. Your Insurance Company requires that we see your insurance card and collect all co-payments at the time of your appointment. If you have any questions about your co-payment, please contact your insurance company.
INSURANCE:
If your insurance requires an “Authorization Form” or “Referral Form” for your visit with us YOU must obtain it prior to your visit. If you do not have your“Authorization Form” or “Referral Form”,YOU will have the option to reschedule or pay for the visit at the time service is rendered and file with the Insurance Company yourself. All HMO and TennCare insurance normally require “Authorization Form” or “Referral Form”. Your PCP can fax these to us at (615) 936-1728 prior to your visit. If you have any questions Please call (615) 936-2020.
PARKING TICKETS:
Effective 01/01/06, all parking tickets will be Electronically Validated when you complete your visit and Check-out. If you leave after 5:00pm,validation is not needed andparking is Free.
