Please send or bring this form to your booth coordinator by the May 9, General CV meeting. Booth Coordinators should give these to Jennefer Harper, 541-514-9487, , or bring them to the Eligibility meeting on May 14, 7pm at the Growers Market building, upstairs, 454 Willamette St. The eligibility meeting is open to all village members. The form must arrive in time for the eligibility meeting. Mailing it to the Community Village may mean that it will not reach your booth coordinator by the deadline. You are responsible.

Each group is permitted a maximum of: 2 worker passes {wristbands} List all requests for Significant Other passes Or 1 worker pass and 3 day passes OR 6 day passes List all requests for Teen {age 13-18} or Youth passes {age 11-12} during Fair Check if participant wants their contact info added to the CV database List all requests for Day Passes, including names if possible Please list all representatives in your group, including yourself List all requests for vehicle stickers; specify type.
