ANNEXURE (FOR THE FELLOWS OF MS/MSC PROGRAMS SPONSORED BY PAEC)
PLEASE BRING THE FOLLOWING DOCUMENTS WITH YOU:
1. DEED FOR LOCAL TRAINING: It should be typed on two or three stamp papers of the total value of Rs.100/- Please do not type the ‘Deed’ on the backside of the stamp papers. The stamp papers must be got issued in the name of the fellow only. The name of the fellow and his/her father must be clearly written on the stamp papers and duly stamped/signed by the stamp vender. The required documents must be got attested and attached with the ‘Deed” as mentioned at the end of the specimen of ‘Deed’.
2. MEDICAL FITNESS CERTIFICATE: A letter in this respect is enclosed. Report to your nearest PAEC medical center/hospital for the said purpose. Original Medical Certificate should be submitted at the time of joining PIEAS.
3. ALL ORIGINAL CERTIFICATES/DMCS/DEGREES: From Matriculation to final degree examination, domicile certificate and national identity card.
A. O AND A LEVEL EQUIVALENCE CERTIFICATE: Those who have studied ‘A’ and ‘O’ levels should provide equivalence certificates of marks percentage that they have obtained.
4. PEC REGISTRATION CERTIFICATE: All those having degrees in engineering discipline should bring Pakistan Engineering Council’s registration certificate. Provision of the said Certificate is must for the grant of fellowship. No stipend will be paid till provision of the same. Those who cannot provide the PEC certificate at the time of joining should submit a proof from PEC that degree has been accredited by PEC
5. HOUSE JOB PMDC REGISTRATION CRTIFICATES: The fellows of MSc (Radiation and Medical Oncology) should also bring house job certificate and registration certificate from PMDC.
6. CHARACTER CERTIFICATE: From the Head of the Department of the University / College last attended.
7. MIGRATION CERTIFICATE: for admission to PIEAS may be obtained from the University last attended.
8. Two sets of the copies of all the academic documents mentioned above (Sr. No. 2,3,4 & 5) duly attested by an officer of grade 17 or above must be provided at the time joining PIEAS
9. Eight passport size photographs with your name and your father's name on the backside of all the photographs.
10. Rs. 6500/-(in cash) for mess advance.
PLEASE CAREFULLY ALSO NOTE THE FOLLOWING POINTS
A. This offer is valid only if you satisfy the eligibility criteria for admission at PIEAS. In case you have foreign degree(s)/certificate(s) you must submit proof of their equivalence as per the Eligibility Criteria of PIEAS.
B. The degree will be accepted only if the concerned university is recognized by HEC
C. Those having foreign medical degree and have done house job from foreign must produce a certificate from PMDC that House Job done is equivalent to one year Pakistani House Job.
D. Fellows, whose results are not declared by 06-11-2017, should submit a certificate from the Controller of Examination clearly indicating that your examination has already been held and viva or project work has also been completed and they are only waiting for result notification.
E. To be able to formally register in the first semester at PIEAS, it is must for fellows to pass the Zeroeth Semester.
F. Please fill up and return the enclosed 'Acceptance' form as soon as you receive this letter.
G. In case your documents are complete, the first payment of the Fellowship stipend to fellows will be made tentatively by the end of December. You are therefore, advised to bring some money with you to meet your essential expenses during the initial period.
H. Your case has been forwarded for security clearance. This is a routine procedure, however if there is any adverse report against you, it could lead to the termination of your fellowship.
I. The fellows are provided shared furnished accommodation in PIEAS Hostels at Nilore. Female fellows are also provided accommodation in Female Hostel. Bed and Mattress are provided in hostel. However, fellows are required to bring their own bed sheet, pillow, blanket and Lenin etc. Since temperature during the winter can goes down to the freezing point, you are also advised to bring warm clothes. The Hostel has a games-room with facilities for several indoor games. Outdoor sports facilities include tennis, badminton and volleyball courts as well as fields for cricket and hockey. Telephone service is provided to the residents of the Hostel on payment. A post office and a bank are also situated nearby. Quite a few shops exist in the Nilore area to cater to everyday needs of the residents. Meals are prepared in the hostel mess on a co-operative (no-profit, no-loss) basis. Students have to pay for the food. The location of Nilore, close to Lehtrar, Murree and Margalla Hills, is very conducive to hiking. Nearby places in Rawalpindi and Islamabad such as Rawal Dam, Shakarparian and the National Park offer some excellent spots for walks, picnics and relaxation. Excursion tours are arranged on a regular basis to places of interest in the surrounding areas such as Murree and Abbottabad.
