All Orders are due NO LATER THAN July 12, 2013. Shipments will begin by October 31, 2013.

Imprint Proof Information:

All default custom imprints are in black Ink only. If you would like Pantone 485 Red to be used in your imprint for your Distributor or Business Unit name Lockup, so you have a two color imprint, the cost is an additional one time charge of $142.00 per each Calendar type you order. Please select this option, if desired, under the Bill To section on page 2.

**You will receive an email to confirm your order has been placed within 72 hours of receipt.
Proofs of your custom imprint will be provided in a PDF file via email for your approval.

Note: If you ordered the 2013 edition of the Desk, Wall or Monthly Planner Calendar, and you would like us to email you the imprint proof from your 2013 edition in order for you to review and make modifications for your 2014 edition, please check this box. You will then not be required to fill out the below imprint information.

-  Default shipment method for Calendar shipments for United States delivery is via Ground Service. Default shipment method for Canada and Mexico customers is via Ground Service where available, and most economical service when Ground is not available in certain areas*.

-  We reserve the right to ship large North American bulk shipments via Truck Line Services*

-  International Shipments are sent via Air Freight – FedEx or UPS*.

All Custom Calendar Orders are to be paid via Purchase Orders. Your purchase order must be issued at the time of order placement. Terms are Net 15 from invoice date. Invoicing occurs after product shipment. **If your order ships to US States of KY or IN, you will be advised as part of the order confirmation if you are required to pay KY or IN Sales Tax.
