1. List all research support with dates (including Sesquicentennial Fellowships and Center Appointments) of over $500 in value awarded to you by the University from 2011 to the present (include support already granted for 2013-2014 and 2014-2015).

5. Describe your project, noting particularly its objectives, significance and the essential materials, methods and procedures, theories and assumption(s) which may be involved. The description should be free of jargon and intelligible, not only to specialists, but also to readers who may not be in your discipline. If yours is a larger multi-year project, please make clear what portion of the project you intend to complete, should you be awarded a summer stipend. (2-page limit; double-spaced; 12-point font)
6. Indicate the current status of your project--that is, whether planned, partly completed, or near completion.
7. If you have already received a summer research grant for this project, please describe your progress on the project since the last award.
8. If you receive the requested award, can your project be completed during the summer of 2016? If not, when, and by what means would you expect to bring the project to completion?
9. If you anticipate any other commitments for the summer of 2016, please specify, and indicate how you would schedule your time.
10. Please list your publications.
11. Please give the names and addresses of two scholars, local if possible, who would be professionally competent to appraise your project, in case the Committee should require further information.
12. What are your plans for publication of the proposed research?
13. Please list other funding being sought for this research project including dates and dollar amounts.
14. State anything further which you think would be useful to the Committee.
