Population Health Data / Review the status of population health data – Network of Care
Elke introduced Joseph Pollard, Division Data Analytics Manager, who discussed the potentialsolution to distribution of population health measures to the RCs that were unable to be part of the SHIP data analytics contract.
The Division is currently moving forward with the Network of Care (NoC) site as the solution and are currently developing a mock of the site to bring back to the January meeting. The health districts already use NoC. Maryland uses NoC for their SHIP. / Identification of population health measures and how they will be displayed / Shaw-Tulloch

Definition of Population Health / Need to come up with a definition of population health
(Handout) There was a robust conversation around the need to create a definition of population health. If a definition were created, should the group consider several definitions, utility beyond the PHWG, etc. PHWG reviewed adocument prepared by Elke as a discussion starter, including definitionssubmitted by workgroup members, definitions used by healthcare, an article on using multiple definitions of the phrase, and a chart showing CDC’s three “buckets” in which the concept of population health is described.
Ultimately, the discussion centered around keeping the definition very broad to mean the broader population versus the clinical subpopulation. However, for the benefit of the regional collaboratives there should be some discussion about how a clinic affects their subpopulations of patients that then impact the broader population. Population health goes beyond the primary care provider to all who are touching the patient.
This item will continue to be worked upon and proposed back to the PHWG via email and will be discussed at the meeting in February. / Create common vernacular

How we use data…. / Describe the PNAS mortality findings for the 45-54 yo age group
(handout) Dr. Hahn discussed the recent study released by showing a trend in high mortality rates in the middle age white population. She also reviewed the same population and rates for Idaho. Findings are very similar to the national findings. The purpose of this presentation was to provide for the type of information that might ultimately be useful for the regional collaboratives. / Provide example of data to support the work of the RCs / Hahn
Update on Regional Health Collaboratives and Virtual PCMH / Review the regional health collaboratives status, community health workers, community health EMS, and telehealth initiatives
Miro and Mary each provided brief updates on the following:
CHW - subcommittee has one-pager. Recommended adopting and adapting MA curriculum, with in-person training. Required core ~ 65 hrs with optional health specific modules (on-line). Proposal was accepted by IHC.
CHEMS - agreement with Ada County Paramedics for curricula, readiness assessment. Measures design workgroup is convening 12/22/15 to identify metrics. SME coming from TX
Telehealth Expansion Committee -- for SHIP improving access to Behavior health and specialty care within the PCMHs. IHC will discuss this at December meeting
Regional Collaboratives - are getting underway with most having meetings in January
Briljent on-board so contact with clinics will need to be coordinated / Knowledge about the status initiatives / Barac

Member Sharing / Oral Health Perspective
- Willing to provide names of active dentists to RCs for potential membership
- HD1 - RC has met once, includes dentist; will have phone call after PCMHs named; and are fully staffed
- HD2 - Executive Leadership team has met; postponing full meeting until clinics selected
- HD3 –RC will meet in January
- HD4 –RC will have a full meeting in January and are looking forward to guidance on population health and the role of the RC
- HD5 - ExecutiveCommittee has met and the full group still being pulled together
- HD6 –Executive Committee has met and the district is now fully staffed
- HD7 –Executive Committee has met and is waiting for clinic identification to establish the whole RC
- Kymberle - explained that SHIP managers will be copied on letters of acceptance to each clinic selected for PCMH transformation
Next Meeting Agenda / Discussion about future agenda items
- Include Briljent/HMA in pop health definition discussion so that we are all working off the same definition
- Karen V. - will talk about FMRI pilot including food insecurity questions
