JOB DESCRIPTION
TITLE:Wellness Coach
LOCATION: Based at Changes Office in Burntwood
HOURS OF WORK: 25 hours per week
Set weekly rota but will require some flexibility
HOLIDAY: 20 days, plus Bank holidays and Statutory Holidays, accruing 1 day per service year up
25 days (Pro rota)
SALARY : Salary £12,922 to £13,257 p.a. for 25 hours
RESPONSIBLE TO: Operations Manager
CONTRACT: Fixed term – 1 year
TRAVEL: Casual car users allowance or second class public transport fares will be payable for travel incurred in carrying out work commitments
JOB SUMMARY:Promotion, Coordination and Delivery of a range of wellness services for adults recovering from mental distress.
DUTIES AND RESPONSIBILITIES:
1) Work with relevant Changes staff and partner organisations, to identify service users who would benefit from engagement with Changes’ services
2) Co-ordination and delivery of Changes Wellness Workshops – including an introduction to Changes 12 steps
3) Setting up and establishing weekly peer-support groups
4) Working with other paid and volunteer staff to coordinate peer-support groups
5) Supporting users to develop and implement customized wellness action plans (WAPs)
6) Use WAP to identify and navigate towards relevant information and public, voluntary, and private sector ‘service providers’
7) Coordinating and overseeing social activities
8) Adhering to appropriate systems of management, monitoring and evaluation
9) To ensure that equal opportunities are fully implemented in all aspects of the service
10) To undertake such other duties commensurate with this post as may be agreed between the post holder and the Executive Committee
This job description will be reviewed annually and may be amended, following discussion with the post holder, to take account of the changing priorities and requirements of Changes Health & Wellbeing.
