Reports to: Living Well (McMillan) Manager/Director of Care
Work base: Penzance
Salary: £17,565 per annum to be paid monthly
Hours of work: 37 hours per week Monday to Friday
Purpose of the job
This is an innovative role working collaboratively as part of a local integrated prevention team. This specific role aims to improve the coordination and quality of care resulting in improved outcomes primarily for people affected by cancer, their carers, families and friends. The post will be part of the Penwith partners including cancer services, social care, community nursing and GPs.
The purpose is to offer a continual supportive presence, not to duplicate the services provided by other agencies, and to ensure that services and support are delivered in a personalised, coordinated, relevant and timely way. This role is crucial to the delivery of personalised planning and will offer encouragement and support for individuals to self-manage and self-care using the best practice available.
The nature of this post will require travel within a specified area therefore a requirement of this post will be access to and the use of a reliable car and a current driving licence.
As duties and responsibilities change, the job description will be reviewed and amended in consultation with the post holder.
This role will be subject to a Disclosure and Barring Service check.
Responsibilities
· Manage and help to train volunteers working as part of the locality team, to offer initial visits, guided conversations and “checklist” reviews to individuals identified within the programme.
· Develop an action plan, liaising with relatives, carers and other involved agencies, as agreed with the individual, to ensure an integrated and personalised approach to care and support.
· Explore and build social networks for the family, offering and developing a range of activities which may include carer support, peer support, befriending, exercise, activity, and cognitive stimulation opportunities, also creativity, enabling patients to pursue new and old hobbies.
· Work with team members and other local statutory, voluntary and independent organisations, to ensure referrals are made appropriately, timely and to respond accordingly, maximising the availability of local resources.
· Understand and be part of a range of specialist clinical and support teams who can provide assistance to an individual and their carer or family which may include assistive technology and equipment, handyperson, hospital from home services, social care, health promotion, speech and language, therapy teams, hydration specialists, nutrition and weight specialist, falls teams, dementia and other long term conditions support.
· Work as part of the Living Well team to motivate and encourage older people to improve mobility and become, and remain, more physically active through involvement in community services and social activities.
· Liaise with other Age UK Cornwall’s services and ensure appropriate links are made to Transport, Home Support, Footcare and others.
· Assist in providing accurate and timely performance reporting within the framework agreed by partners.
· Understand and assist to identify triggers and develop protocols for ensuring any deterioration results in an appropriate and timely response.
· Encourage people and members of the community to record their experiences of Health & Social Care via Healthwatch Cornwall in order to improve their services.
· Provide cover for the Living Well Coordinators within the Penwith Living Well team.
· Promote the work of Living Well and Age UK Cornwall & The Isles of Scilly at every opportunity.
Macmillan Cancer Support
· Work with Macmillan Cancer Support, as part of the Macmillan project in Penwith and the Isles of Scilly, to provide support, information and signposting to all relevant services to people affected by cancer; liaising with appropriate healthcare professionals, Macmillan local services and other relevant local, regional and national organisations.
General
· Attend mandatory training (including Macmillan training) and meetings as directed by the Line Manager.
