JOB TITLE: CASE MANAGER
SUPERVISOR: EXECUTIVE DIRECTOR
GENERAL DESCRIPTION OF POSITION:
The Case Manager is responsible for providing all case management designed to move guest families through the Family Promise Program and into permanent housing, by providing compassion and support.
JOB DUTIES AND RESPONSIBILITIES:
Job duties and responsibilities are as assigned by the network director and may include, but are not limited to:
- Develop and implement all policies regarding case management, including screening, intake and exit of guests from program.
- Screen all prospective program guests using telephone and in-person interviews.
- Perform guest intake process to include orientation into the program, needs assessment, and development of an individual case plan (to include steps and timetable).
- Perform guest exit process to include exit interview, moving plan, and development and implementation of a 24-month follow-up plan for all program graduates.
- Create and manage a support and information system for and track the success of guests who have exited the program.
- Conduct weekly case meetings with each guest family to review case plan, track progress, set goals, deal with any issues, and provide guidance, counseling and needed resource referrals.
- Maintain up-to-date records on all guests served and keep necessary statistics.
- Direct and assist guests in daily Day Center tasks in regard to fax, computer and other resources.
- Schedule guest transportation as needed and provide transportation services using the Network Van (15-passenger) when needed.
- Identify and secure community resources for guests.
- Provide on-call after hours coverage as assigned by the Executive Director.
- Develop and implement all in-house programming for guests, which may include financial, parenting, nutrition and life skill classes.
- Ensure compliance of all guests with program guidelines and report violations to the Executive Director for immediate follow-up.
- Maintain effective communication with the Executive Director by providing weekly written reports on each family and a monthly written case management report. Meet regularly with the Executive Director to review guest issues, progress, resource needs, training needs, issues that need to be addressed, etc.
- Attend meetings with collaborative social service partners to present FPI information and gather resource information for FPI staff and guests.
- Provide report (basic update on current and past guest families) for monthly Coordinator Meetings.
- Participate in at least one training or educational opportunity per year in order to become more proficient on the job.
- Assist other staff in providing training for host volunteers as needed.
- Perform other duties as assigned.

DESIRABLE QUALIFICATIONS:
· Bachelor’s degree in social work or related field or three years experience in a similar position.
· Any work and/or life experiences which contribute to maturity, sensitivity to others and motivation to work in a social service agency is helpful.
· Demonstrated experience in crisis intervention, problem solving, and resource allocation.
· Passion for the program and its goals.
· Valid driver's license and good driving record.
