MAIN PURPOSE
To provide full support for an individual with an acquired brain injury and complex needs within his family home, to allow him to lead a meaningful and active life. To work as part of team providing 24 hour 2-1 staffing during the day and throughout the night. In addition to support the individual to visit his caravan near Chester one week per month on average between the months of April and September. To provide consistency of support when individual is admitted to hospital

Salary/Hourly Rate: £7.50 Unqualified, £7.60 Qualified

Annual Leave
80 hours per year. In addition to eight paid statutory bank holidays (bank holidays are pro rata for part-time staff).

PSS operates an Occupational Pension Scheme with the Pensions Trust.
Conditions of Service
Offers of employment are made subject to the following conditions:
a) Completion of medical questionnaire
b) Receipt of two satisfactory references, including one reference from a current or most recent employer.
c) Authorisation to work in the UK. Under the Immigration, Asylum & Nationality Act 2006 successful candidates will be required to produce documentation which demonstrates their authorisation to work in the UK. Documents may include; Passport; full birth certificate; UK work permit; Home Office letter.
d) An Enhanced Disclosure. Please note that having a criminal record does not automatically disbar you from working with PSS.
e) Proof of application to register with the General Social Care Council (GSCC). From April 2005 all qualified Social Workers who wish to use the professional title will need to register with the GSCC.

Service Delivery
Get to know the individual
Know the individual’s likes and dislikes and what is important to him. Supporting the line manager to complete a person centred plan with clear outcomes, including:
· His life story
· How he likes to spend his day
· How to support contact with his family and friends
· How to support his social life and accessing community resources
· Understand his feelings when he becomes distressed and know how to support him to feel better
· Understand how he communicates – he communicates through changes in mood and expression
Be accountable for your own practice
Understand and demonstrate awareness that your practice has an impact on the people you support
· Be compassionate, considerate and kind
· Be respectful of the individual and his family’s right to privacy, dignity and security and respect his feelings
· Promote their rights, choices and wishes
· Communicate effectively with him making sure that you actively observe and respond to him appropriately
· Work with him at his pace and provide clear explanations about what you are doing
· Communicate effectively with him, his relatives, other colleagues and health and social care professionals
· Complete regular observations of him including blood pressure monitoring, oxygen levels and physical changes
· Ensure that information and changes in his needs are acted upon in a timely manner
Empower individuals using the service
Know what he can do and use your skills to support him to do it
· Encourage him to be active both physically and socially by providing a diverse range of activities within the scope of his capabilities and advice from Healthcare Professionals
· Support him to communicate his wishes
· Represent the individual and his family during hospital admissions, appointments and accessing activities
· Support him with nutrition and hydration – the individual is currently PEG fed
· Support him to maintain his health and well being
· Support him to manage his personal hygiene by completing daily personal care tasks
· Support him to succeed in the things he would like to do
· As much as possible do things with him - not for him or to him and promote his dignity and choice
· Help him to understand his environment and the use of his senses including sight, hearing, smell, and touch.
· Support him to manage and maintain property, by performing regular health and safety checks and liaising with his family
· Complete household tasks on his behalf including maintaining the tidiness and cleanliness of his home
· Support individuals to manage his finances in line with their personal plan under direction from his family
Comply with legislation and organisational policy
· Keep accurate records of your time spent with individuals capturing the information that counts
· Ensure full handovers between shifts are completed
· Ensure that you are fully aware of day to day changes and developments within the service by keeping abridge of records
· Maintain written records and complete documentation in compliance with appropriate policies and legislation
· Ensure that policies and procedures set by PSS are maintained
· Assist him and his family in the safety and security of the dwelling
ADDITIONAL DUTIES
To actively participate in developing directorate, team and individual objectives and plans, attend staff performance management and learning and development reviews.
The post holder is expected to be flexible in the performance of duties and to undertake any other duties identified as appropriate to the post.
