Departure Checklist

This form is to assist your staff member in leaving Otago Polytechnic. You (the manager) must complete the departure checklist prior to your employee’s last day at work, sign it as completed, get your employee to sign it and send it to the Human Resources Department.

Advise ISS computer staff to close network and email account. If your data (email, mailing list) is to be used by your replacement person advise ISS accordingly. Please copy or transfer files to shared directories (Access to directories/email will cease the day after your last day of employment) / (extn 8039)
Human Resources & Payroll (HR)
Original resignation letter sent to Human Resources / Human Resources, Room F207
Final Payslip is available on ME@OP (ME@OP access if available 2 weeks after final day of Employment). ** / (extn 8022 or 8007)
Arrange Certificate of Service – (prepared upon request if required) /
Notify HR of forwarding address for any correspondence / (extn 8007)
Manager to notify Cathy Morris, Coordinator in Customer Services and Stuart Paine, Accounts Receivable Officer to ensure all staff debt is paid / (extn 8049)

Security
Your swipe card and keys should either
(a) be returned to the manager; or
(b) be returned to the Campus Services Department / Campus Services (extn 8387)
Before sending swipe cards in the internal mail, email (extn 8387), and request that the card access be cancelled
Return Staff ID card to Human Resources / Room F207
Return any other work-related cards / Return to Manager
Otago Polytechnic Property
Return any property OP has supplied to you, e.g. books, resources, cell phone, laptop, PC at home, car / Return this property to the person who issued it to you. Ensure the handover is documented
Return any occupational health aids supplied to you / Return to Health & Safety Manager

Library – check all books are returned / Main reception area of Bill Robertson Library ()

** Manager to advise final pay is in the normal payrun on the next available pay period.
