Position:Care Coordinator
Department:Wraparound Care Coordination
Work Hours:9am – 5pm, Monday through Friday. Schedule may fluctuate based on client needs
Status:80-hours per pay period
AJA Counseling Center is currently recruiting for five full time Care Coordinators
Brief Job Description:
Provides case management services to a caseload of 8 families with the focus of assuring that children remain with their family and in their community. Provide weekly face-to-face contacts with the youth AND family in their home or in the community.Manage crisis that may occur within the family.
Assembles child and family teams; assesses strengths and needs of the family unit; coordinates meetings; seeks community resources for assigned families; and completes all necessary documentation
Within the first 6-months of your becoming a Staff Member at AJA, you are required to become certified to provide case management to Wraparound-Milwaukee by completing 54-hours of mandatory training in Wraparound philosophy and policies, which isprovided by Wraparound-Milwaukee. The Wraparound Model is an evidence based practice model.
Qualifications:
MUST HAVE - BA or BS degree in Social Work, Psychology, Nursing, Occupational Therapy, or related field with experience in Human Services, preferably Case Management
Must have a vehicle, and maintain a driver’s license and adequate insurance coverage
A minimum of 1-year of demonstrated administrative capabilities
Interpersonal skills necessary to deal effectively with a diverse group of staff, residents and community
Strong computer skills as all information is documented in electronic format
Analytical skills to gather and interpret data and find solutions to difficult problems
Ability to work effectively during crisis situations
Proficient in use of computer systems, to include Microsoft Office, Word, Access, and Excel
