6 BRIEF RESUME OF THE INTENDED WORK

6.1 INTRODUCTION:-

Ayurveda –The science of life deals shodhana & shaman as treatment entities. Panchakarma-The prime shodhana chikitsa not only helps to cure the disease from its root level but also prevents its reoccurrence. Rapid urbanization, sedentary life style and stress are the causes of various diseases. Incompatable foods and activities lead to formation of Ama as well as vitiate vata by which the diseases like Amavata are manifested.

Some signs and symptoms of Amavata are described in modern text books under the heading of Rheumatoid Arthritis in which systemic features such as symmetrical joint pain, fatigue and diffuse musculoskeletal pain are manifested associated with swelling of joints. The prevalence of R.A is estimated to be 1.2%. Out of 60% of the population having some form of arthritis by the age of 60. It is three times more common in women than in men. R.A is very common in India, particularly in Northern states like Punjab and Haryana. Osteoarthritis, usually seen in old age but RA affects younger people also. It has been seen that people between the ages of 20-50 years are susceptible to the disease.

NEED FOR THE STUDY:-

In Ayurveda for the management of Amavata several medicaments are mentioned. As Amavata is pain dominating and movements restricting disease, it hampers the normal activities of person.The modern medicines like NSAIDS, Steroids ,sedatives etc gives temporary relief from pain and stiffness but some side effects are manifested. So there is need to research for a effective long lasting treatment. Ayurvedic treatment fulfills this criteria. By basti Chikitsa which is considered as ardhachikitsa of chikitsa, can give relief to such diseases. That’s why the present study based on role of basti in the management of Amavata.

6.2 REVIEW OF LITERATURE:-

The term Amavata is not described as a disease entity in brihatrai but described in detail in laghutrai. Acharya Vangsen, Madhavkar, Chakradatta and Bhava Mishra described the Etiopathogenesis, Symptomatology and treatment in detail in similar fashion.

NIDANA & SAMPRAPTI[1] a

Due to unaccustomed foods and activities, poor digestive fire, daily intake of fatty foods ama is formed and circulates through the dhamanis, propelled by vata and associated with kapha, obstructs the channels to give rise to weakness of digestive fire and feeling of heaviness of the body. This ama becomes the causative factor of amavata.

LAKSHANA [1]b

Pain all over the body, loss of taste, thrist lassitude, feeling of heaviness of the body, fever, non digestion of food, swelling of the body parts – are the symptoms of Amavata.

CHIKITSA SUTRA [2]

Langhana, swedana, use of bitter drugs and pungent tastes, stimulating hunger, purgation, and enemas are the treatments of Amavata

6.3 OBJECTIVES OF THE STUDY:-

1) To study role of basti Chikitsa in Amavata in detail.

2) To Assess the efficacy of Prasarini taila basti in Amavata.

7. MATERIALS AND METHODS:-

7.1 SOURCES OF DATA: -

  1. The study is a clinical trail hence the patients will be selected from Shri Siddharoodha Charitable Hospital and Research Centre attached to N.K.J Ayurvedic Medical College, Bidar &other associated hospitals in the Bidar city.
  2. Ayurvedic classics, journals, magazines, seminars, conferences, digital library and internet.

TRIAL DRUGS:-

For the present research Study Prasarini Taila is selected.

Contents of Prasarini Taila : - Eranda Taila and Juice of Prasarini[3]

  1. Raw drugs will be selected from the Ayurvedic pharmacy under the supervision of Dravyaguna specialist
  2. Under the supervision of the Rasashastra expert of this institution trial drug will be prepared.

7.2 METHODS OF COLLECTION OF DATA:-

30 patients will be selected by simple randomized sampling procedure by applying inclusion & exclusion criterias and will be divided in two equal groups. Standard case sheets will be prepared by incorporating signs and symptoms of amavata. The special format of basti karma will be made to observe the effect of therapy.

SELECTION CRITERIA:-

INCLUSION CRITERIA:-

  1. Patients of age group between 20 to 60 years irrespective of sex will be taken
  2. Patients presenting with signs and symptoms of Amavata mentioned in classical texts
  3. Patients diagnosed as rheumatoid arthritis by various investigations including ESR,

C - reactive protein, RA factor, X-ray, ASO titre.

EXCLUSION CRITERIA:-

  1. All the conditions where basti is contraindicated.
  2. Patients with systemic pathologies like cardiac diseases, renal diseases, SLE etc.
  3. Patients with tuberculosis of spine, spinal tumours, vertebral fractures, connective tissue disorders & surgical conditions.

4. Chronic patients like swan neck and ulnar deformity.

RESEARCH DESIGN:- Trial Group :- 15 patients will be administered Prasarini taila basti in the form of yoga basti.

Control Group: - 15 patients will be given Amritadi choorna 10 gm BD with rice wash water

for 8 days.

Contents of Amritadi Choorna : – Amrita, Nagara, Goksura, Mundatika and Varunaka[4]

FOLLOW UP: - There will be two follow ups in both the groups after 16 days and 32 days.

The progress will be noted on the basis of assessment parameters (both subjective and objective), before treatment & after treatment in a specially prepared case sheet.

The observation will be analyzed critically & scientifically by employing some statistical test between the groups with suitable statistical analysis
