It is recognized that many professional development activities are related to multiple categories and do not fit neatly into any one category. However, rather than repeating your activities and accomplishments in multiple categories, please choose the best-fitting category for each activity and document your activities there. (It may help to reference your personal statement when selecting in which category to place an accomplishment. For example, if your career goals include teaching and you have an activity that can fit into the teaching or outreach/practice categories, then you should list the activity under teaching.)

Please note: We have included all of these categories and examples to be inclusive and allow for diversity in accomplishments and activities—you are NOT expected to have accomplishments to list in every category! If you have nothing to list in a specific category please write N/A.

1.  Research

a.  Briefly describe the progress you have made on your thesis (M.S. or Ph.D.) since your last annual report. Include a brief description of your area of interest or specific topic for your thesis or dissertation.

b. List and briefly describe your role in any research (other than your thesis/dissertation research) in which you have participated since your last annual report. Describe your specific responsibilities and accomplishments. Be sure to indicate if you had a leadership role in any project(s).

c.  List any authored or co-authored papers or posters submitted, reviewed, or accepted for publication or conference presentation. Indicate whether it was peer reviewed, the status and outcome of the submission, and your contribution to the paper or poster.

2. Teaching

a. List and briefly describe any teaching activities or assistance in teaching activities in which you have been engaged since your last annual report. Include any guest lectures, brown bag presentations, formal or informal mentoring of other students, etc. Attach copies of any relevant evaluations of your performance (letters, student evaluations, etc).

b. Indicate participation in any formal or informal teaching development activities. For example: supervisory sessions with an experienced teacher, workshops, or training sessions.

3. Outreach/Practice

a. List and briefly describe any outreach or practice activities in which you have been involved since your last annual report. For example: Gave a workshop in the community, collaborated on a community project, disseminated research-based information to a community audience, organized a conference/summit/collaborative meeting, helped develop a university-community partnership, designed or implemented an intervention or prevention program, or provided counseling or consultation.

b. List any dissemination products (print, internet or other) you have authored or co-authored and your role.

c. List any community-based, outreach/practice grants submitted or obtained and your role.

4. Leadership/Service

a.  List and briefly describe any formal leadership roles you have had in the department, university, community, or profession since your last annual report. For example: Chairperson of GSO, student editorial board of a journal, student representative on department or university committee, board member of a not-for-profit, or member of a committee in a professional organization.

b. List any informal roles or activities you have engaged in that support the department, the school, the university, your profession or your community. For example: Reviewed article for a journal, volunteered as contact person or host for prospective students, organized a brown-bag or a departmental social event, attended job talks and provided input to the faculty, or contributed to a departmental newsletter or website.

5. Learning

a.  List any content area or skill development learning activities you engaged in related to your professional development goals since your last annual report. Include title and date of activity. For example: conference attendance, workshop participation, training sessions, auditing a class, learning new software or statistical programs, development and practice of a specific professional skill, learning a coding system, writing for a new audience, attending HDFS brown bags and colloquium, attending other university talks, or receiving career counseling.
