JOB TITLE: Livewell NHS Health Checks Advisor

2. PURPOSE OF JOB

The Livewell programme is a holistic healthy lifestyle programme, which addresses health inequalities through an offer of NHS Health Checks, motivational interviewing, physical activity, nutrition, stop smoking services and health education.
The NHS Health Check programme aims to prevent heart disease, stroke, diabetes, kidney disease and certain types of dementia. The NHS Health Check assesses the risk of developing these health problems and gives personalised lifestyle advice on how to reduce it.
The role of the Health Check Advisor is to support in the development and delivery of an innovative and create way to deliver NHS Health Checks within local communities to ensure services reach vulnerable groups. In addition, you will be expected to ensure there are strong links between NHS Health Checks and the wider Livewell service which will support long term behavioural change.

4. MAIN RESPONSIBILITIES

1.  To deliver NHS Health Checks in the community, focussing on those from deprived communities which will contribute to helping reduce health inequalities in the city.
2.  To increase awareness and up take of the programme and interventions amongst target populations, reducing inequalities in uptake, access and outcomes.
3.  To have a clear understanding of the principals of NHS Health Checks and an ability to convey the nature, risks and benefits associated with a NHS Health Checks to clients
4.  To perform point of care testing and record routine clinical measurements, using appropriate tools and methodologies to measure individual’s physical indicators of risk of cardiovascular disease in accordance with the approved protocols to ensure performance and quality.
5.  To be competent in how to assess someone’s cardiovascular risk, interpret these results and communicate them effectively to the client.
6.  To help identify community groups and target audiences to increase the uptake of NHS Health Checks.
7.  To work with local communities to understand their issues and concerns around NHS Health Checks, using health promotion and community development approaches as appropriate to develop joint solutions.
8.  To organise and attend outreach events to deliver services within local communities.
9.  To work in line with the Code of Conduct for Healthcare Support Workers and Adult Social Care Workers and adhere to the standards of conduct, behaviour and attitudes that are expected.
10.  To work in partnership with the wider Livewell service in order to signpost eligible clients to the service and other services where deemed appropriate
11.  To maintain accurate and efficient records of client data in line with the Data Protection Act and provide monthly retention figures and outcome reports.
12.  To work within the monitoring and audit tools for the service and prepare progress and evaluation reports to monitor the effectiveness of the programme through appropriate data collection both quantitative and qualitative.
13.  To undertake professional training on key issues like behavioural therapies in line with national standards and local requirements to ensure that personal practice meets the required quality standards.
14.  To work within a rota system/shift pattern to meet the needs of the service
15.  To attend team meetings and supervision sessions as required
16.  To respect the individuality, values, dignity and diversity of individuals and contribute to the provision of a service sensitive to the needs and rights of people in the community.
17.  To exercise good personal time management, punctuality and consistent reliable attendance.
18.  To perform duties to the highest standard with particular regard to effective and efficient use of resources, maintaining quality and contributing to improvements.
19.  To undertake a range of other duties as required from time to time ensure the efficient and effective operation of the service.
20.  To maintain a working knowledge of IT and its development within the team.
21.  To ensure compliance with health and safety legislation and council policy.
22.  To apply appropriate health and safety measures and standard precautions for infection prevention and control relevant to the test procedure and environment.
23.  Adhere to the NHS Health Check competence framework
