Notice of Availability of Grant Funds
Intensive Outpatient Treatment and Support Services
Take notice that, in compliance with N.J.S.A. 52:14-34.4 and 34.6, the Division of Mental Health Services (DMHS) hereby announces the availability of the following grant program funds:
Name of program:Intensive Outpatient Treatment and Support Services
Purpose: This notice announces the availability of grant funds to create Intensive Outpatient Treatment and Support Services programs to provide treatment to individuals with exacerbation of the symptoms of mental illness and/or a co-occurring substance abuse disorder through services that include comprehensive assessments, wellness and recovery action plans (WRAPS), medication administration and education, individual therapy, structured group therapy, illness management and relapse prevention groups, family therapy, the provision of or arrangements for physical health care and direct linkage to ongoing clinical and support services.
Amount of funding available: The Division of Mental Health Services is reserving up to$7.0 million to support the development of Intensive Outpatient Treatment and Support Service Programs. Contracts awarded as a result of this
Request for Proposal (RFP) are renewable up to two years, at which time DMHS will review agency outcome performance and make contract continuance determinations.
Organizations that may apply: In order to be eligible for consideration for this Request forProposals, an applicant must be a corporation duly registered to conduct business within the State of New Jersey.
The applicant must demonstrate the ability to meet all rules and regulations for any DMHS program element of service proposed by the applicant.
The applicant must comply with the terms and conditions of the Department of Human Services contracting rules and regulations as set forth in the Standard Language Document, the Contract Reimbursement Manual (CRM), and the Contract Policy and Information Manual (CPIM).
Any fiscally viable corporation, as noted above, which meets the qualifications of the Department of Human Services' Contract Policy and Information Manual, N.J.A.C. 10:3, may apply. If needed, a copy of this manual may be obtained at the time an application is requested. Applicants may contact the Division of Mental Health Services Contract Unit at 609-777-0608 with general questions about the requirements in these manuals.
Qualifications needed by an applicant to be considered:Applicants must document demonstrable experience in successfully providing outpatient mental health services and supports to adults with serious and persistent mental illness who are admitted to the program from specified referral sources and servedin a mannerfully consonant with recovery principles. Applicants must propose a program specific to the needs of the target population in such a way as to support their community tenure, diminish their use of other community based acute care and hospital based mental health services and must demonstrate a strong commitment to consumer choice and consumer directed care.
Proposals must be feasible within budgetary parameters and should include a proposed means to evaluate the program's effectiveness.
Applicants awarded grant funds must provide reports to the DMHS contracting and regional officesthroughout the project period.
