RE: Physicals

The Lehigh Valley Health Network is pleased to provide free sport physicals to all student-athletes in the East Penn School District. In order to provide an easy and effective physical, please adhere to the following directions:

  1. Complete all appropriate sections of the PIAA Comprehensive Initial Pre-Participation Physical Evaluation (PIAA CIPPE Form). In order to be eligible for the physical the following sections must be completed:

Section 1: Personal and Emergency Information

Section 2: Certification of Parent/Guardian

Section 3: Understanding of Risk of Concussion and Traumatic Brain Injury

Section 4: Understanding of Sudden Cardiac Arrest Symptoms and Warning Signs

Section 5: Health History

  1. Section 6 will be completed by the doctors at the physical exam.
  2. All athletes should bring or wear gym shorts, t-shirt and corrective lenses (if applicable).
  3. Failure to report for a sports physical places the responsibility to complete the PIAA CIPPE form on the athlete and his/her parent/guardian.
  4. No student-athlete will be eligible to participate in interscholastic athletic competition (try-outs, practice or games) until his/her CIPPE form has been delivered to the EHS Athletic Office.

2017-18 FALL SPORTS SIGN-UP INFORMATION

  • Practice for all fall sports officially begins on Monday August 14, 2017
  • Every candidate must submit a PIAA Comprehensive Initial Pre-Participation Evaluation Form (CIPPE) to the EHS Athletic Office no later than Monday, August 7th to be eligible to participate in the first day of practice. Please double-check that Sections 1-5 have been completed by a parent/guardian and Section 6 by a licensed physician.
  • The EPSD is offering a free physical on Wednesday, June 7th in the EHS Cafeteria (schedule below):

DUE TO THE LARGE NUMBER OF ATHLETES, STUDENTS WILL NOT BE PERMITTED TO ENTER THE PHYSICAL LINE UNTIL THEIR ASSIGNED TIME.

MIDDLE SCHOOL TRANSPORTATION: immediately following dismissal special buses will be available to students at both schools. All buses will be specifically marked for EHS Physicals. Students are expected to make individual arrangements for transportation home after physicals. No bus transportation will be provided.

PIAA PHYSICAL REQUIREMENTS: students are required to have one (1) sport physical per school year. Sports physicals for the 2017-18 sports year CANNOT be conducted prior to June 1, 2017.

PLEASE NOTE: June 7, 2017 is the only date that fall sports physical examinations will be offered at Emmaus High School. If a student does not receive a physical examination on June 7th, 2017 at EHS, the student must schedule a physical with a private physician. Any physical examination not performed on June 7, 2017 at EHS must be submitted directly to the EHS Athletic Office no later than 2:00pm on Monday August 7, 2017. Athletic Trainers and coaches are not permitted to collect physical forms on the first day of practice.
