Strathcarron Hospice provides Specialist Palliative Care to patients who have complex problems associated with any type of life-threatening illness and also offering support to families/carers. Physiotherapyintervention and care is an integral part of our rehabilitative and multi-professional approach which aims to enable patients to achieve and maintain the highest possible quality of life and where possible to be cared for in the place of their choice.
We are looking for Physiotherapists who can work flexibly on an ‘as required’ basis to enable us to provide consistent cover for our busy services. You should be a registered Physiotherapist with experience in a range of clinical settings. Experience of working with cancer or palliative care patients would be an advantage. You should have an ability to communicate effectively and sensitively with patients and carers, have a passion for patient centred care, be a creative problem solver and have an ability to work as part of a team.
You will need access to a vehicle which will allow you to travel as required.

JOB PURPOSE:
To provide a high quality physiotherapy service to Strathcarron Hospice, the focus of which will be to provide a rehabilitative approach to patient care/ maintaining quality of life.
DIMENSIONS
  • Practices as an autonomous professional.
  • Responsible for the provision of physiotherapy service for the 24 bed Inpatient Unit, 20 patient Day Care Unit and offer advice to the Community Care Team as required.
ROLE OF DEPARTMENT
  • Delivery of holistic physiotherapy services within a multidisciplinary framework.
  • Collaborates with other health disciplines, community services and carers in ensuring a seamless patient focused service.
  • Promote a rehabilitative approach
KEY DUTIES/RESPONSIBILITIES
  • To promote a rehabilitative approach to patients and the multi-disciplinary team.
  • To take clinical responsibility for a designated case load of patients, managing clinical priorities and time efficiently and effectively.
  • To identify and assess individual patient’s symptoms and needs, and implement appropriate, evidence-based physiotherapy interventions. These should closely reflect the intentions of the multi-disciplinary team.
  • To maximise the independence and quality of life for patients for as long as possible in accordance with the patients’ preferences
  • Recognise the changing needs, capacities and wishes of the patient throughout the progression of their illness.
  • To utilise advanced communication and interpersonal skills to negotiate patients / carers co-operation with treatment interventions, taking into account the complexity and range of challenges presented e.g. emotional, cultural, behavioural and other impairment.
  • To communicate a range of difficult and sensitive information to patients and family/carers
  • To show initiative and adaptability in the delivery of treatment and care with individual patients, evaluating and modifying techniques and equipment as necessary
  • To communicate with the hospice multi-disciplinary team and with externally based services/health professionals to ensure the co-ordinated delivery of patient care.
  • To maintain written and electronic professional records.
SYSTEMS AND EQUIPMENT
  • Full range of physiotherapy equipment including moving and handling equipment, walking / mobility aids etc
  • Electrical equipment – Tens, heat pads.
  • Respiratory Equipment – Nebulisers, Suction Units, Oxygen delivery units and BiPAP
  • Manual aids, hoists, sliding sheets, sliding boards, swivel boards, easy rise chairs, arjo plynth.
  • Rehab Equipment – set of stairs, parallel bars, hand weights.
  • Supports – splints, TEDs, Lymphoedema sleeves and stockings
  • Clinical records – paper and electronic systems
ASSIGNMENT AND REVIEW OF WORK
  • Clinical caseload will be generated by specific service needs of each area.
  • Work independently as autonomous clinician in physiotherapy
  • Attend staff and clinical meetings, feedback and contribute to decision-making in multi-disciplinary team.
  • The Senior physiotherapist/Rehab Lead may delegate other specific tasks.
  • Expected to anticipate immediate problems and needs and to take steps to resolve these, involving appropriate colleagues where necessary.
DECISIONS AND JUDGEMENTS
  • Make decisions on appropriateness of patients for physiotherapy treatment based on a comprehensive patient assessment and taking into consideration complex presentations and multiple pathologies. Use advanced clinical reasoning skills to provide physiotherapy diagnosis and appropriate treatment plans. This involves evaluating progress, altering treatment programmes accordingly and referring to appropriate professionals as required.
  • Prioritise caseload and time management.
  • Act as a resource of physiotherapy knowledge to offer advice for a wide range of professionals internally and externally.
MOST CHALLENGING PART OF JOB
  • Work independently without direct support of physiotherapy colleagues or Rehab Lead.
  • Daily working with patients who have complex physical, emotional and spiritual needs.
  • Daily involvement with patients and families in distressing circumstances e.g. terminal phase of illness.
  • Undertake mentally and physically demanding job whilst at the same time taking care to safeguard one’s own health and safety as well as that of colleagues and patients
COMMUNICATIONS AND WORKING RELATIONSHIPS
Patients/Carers:
  • Provide and receives information regarding assessment, diagnosis, prognosis and treatment to encourage compliance, utilising verbal and non-verbal techniques.
  • Required to utilise highly developed empathic, motivational, persuasive and interpersonal skills to facilitate maximising rehabilitation potential.
  • Convey information in a sensitive manner when it is contrary to the expectations of the patient and to support the patient in coming to terms with this information.
  • Deals with verbal complaints.
  • Highly complex medical conditions may result in patients having communication difficulties and significant communication and language barriers requiring highly developed interpersonal skills.
  • Using active listening skills, convey awareness and respond to families’ fears and concerns.
Multi-Disciplinary Team
  • Communicate directly with medical and other colleagues regarding patient condition and effectiveness of treatment plans, ensuring team approach to appropriate interventions and care.
  • Collaborate with members of multi-disciplinary team to facilitate effective discharge planning for and with patients.
PHYSICAL MENTAL/EMOTIONAL AND ENVIRONMENTAL DEMANDS OF THE JOB
  • Manual handling techniques using mechanical (hoist) and non mechanical (sliding sheets) aids.
  • Pushing wheelchairs, moving walking aids
  • Pushing recliner chairs, beds.
  • Physiotherapist will have to stand or work in crouched position, despite complying with moving, handling procedures for periods varying from a few minutes to approximately half and hour for treatment to be carried out (for most of the day).
  • Emotional resilience required to work with patients at end of life / facing multiple losses
  • Imparting unwelcome news regarding the patient’s rehabilitation prospects and breaking bad news and dealing with loss.
  • Communicating directly with distressed, anxious and worried patients and families. Initiating discussions regarding sensitive issues and dealing with grief.

Terms and Conditions:The terms and conditions of service are those laid down in line with the Whitley Council terms and conditions.

Salary Scale:Band 5, £22,440 to £29,034 per annum pro rata

Hours:As required by the service.

Employees may be required to vary the pattern of their working week subject to the exigencies of the service.

Duration:Permanent.

Annual Leave:27 days rising to 29 days after 5 years’ service and to 33 days after 10 years’ service plus 8 public holidays (pro rata).

202.5 hours rising to 217.5 hours after 5 years’ service and to 247.5 hours after 10 years’ service plus 60 hours public holiday (pro-rata)

Sick Leave & Pay:In accordance with Strathcarron Hospice provisions.

Superannuation:This post is superannuable under The Falkirk Council Local Government Superannuation Scheme and your remuneration will be subject to deduction unless you choose to opt out of the scheme.
