Dear colleagues,

My name is xxxxx and I am a member of the steering committee for the Schwartz Rounds. I have worked at xxxx since 2011; I work full time on xxxx ward, an acute neurorehabilitation ward.

I should be most grateful if you would complete this anonymous questionnaire at the beginning and end of today’s session. The aim of the questionnaire is to evaluate the impact of the Schwartz Rounds. The data will be stored in a large password-protected database on a secure server and will only be seen by me. The study has been approved by the trust’s Caldicott Guardian and medical director.
