Application Instructions – 2019

Please fill out relevant information on the application form. Deadline is December 1, 2018.

• Submitter Type: Indicate who is submitting the form, e.g. teacher, parent, etc.

• Age: The age of performer at the submission deadline (December 1, 2018).

• Program: Provide composer’s name as it would appear on the program, full title, including opus and number, and the approximate duration of the piece in minutes.

• Audition Method: Include the full URL (clickable link) to the performance on YouTube, Vimeo, or some other publicly accessible internet site. Be sure to include relevant passwords to video locations if necessary.

• Application Fee: Enclose a check made out to Golden Key Music Festival for the relevant application fee(s). Note: Application Fee is $150 per applicant for applicants within the US and $200 per applicant outside the US. Checks must be standard and payable to the USA banks. If you cannot send a standard check, please contact the Competition Director, George Borisov, at to discuss international transfer options. Include an email address and a telephone number where you can be reached. All fees are non-refundable.

Consent and Release

I, ______hereby consent to be interviewed, recorded, photographed, videotaped, or filmed by representatives of Golden Key Musical Festival (hereinafter ‘GKMF’), which photograph, film, audiotape, webcast, broadcast, stream, record, postings on social media, releases to news media, digital displays and all other forms of media, whether now or hereafter known (hereinafter known as “the Materials”), may be used for any purpose that in the sole discretion of GKMF advances the goals of GKMF. I hereby give GKMF permission to use my name, likeness, and biographical information and to use, promote, or exploit the Materials, in whole or in part, or any derivative work of the materials.

I agree that such recordings, interviews, articles, quotes, photographs, films, audio or video recordings, and/or reproductions of the same in any form are the property of GKMF. I further understand and agree that these Materials may be edited, used, published, distributed, and republished by GKMF, now or at any time in the future, for the purposes set forth above. I waive all rights to approve the use of the materials, now or in the future. I understand that I have no claims (including, without limitation, claims based upon copyright, invasion of privacy, or defamation) arising out of or in connection with any use, alteration, or use in any composite form, of the materials.

I relinquish any present or future claim for reimbursement for said recordings, or photographic or film reproductions of my likeness. I forever release and discharge any and all actions or claims, demands, costs, and liability that may arise from the use of the Materials, which I, my family members or my heirs may have against GKMF and/or George Borisov, GKMF officers, trustees, employees, contractors, funders, agents, or any other third party contracting with GKMF. This Release shall inure to the benefit of the successors, assigns, licensees, and legal representatives of GKMF and George Borisov.

I retain the right to use my participation in GKMF’s recitals in my own public relations, including in all of the media above-described.

I acknowledge that I have read this consent form in its entirety, or it has been read (or translated) to me, and I have had the opportunity to ask questions about it and understand it.

I am 18 years of age or older or am represented in this instance by the Submitter listed and agree to the terms set forth above.
