Field Trip Permission Slip to NHMU
Dear Parents or Guardians:
As part of the fourth grade science and social studies core, we will be going on a field trip to the Natural History Museum in Salt Lake City. This opportunity will help solidify the knowledge of our rocks and fossil units and Utah Native Americans.
Date of field trip: Tuesday, October 18th, 2016
Times: Leaving school at about 9:15 am and returning at about 2:30 pm.
IMPORTANT INFORMATION-
· All students must be in uniform.
· Students must have their home lunch if applicable.
· No other children are allowed.
· All students will be supervised by an adult at all times, both during travel and our visit to the Natural History Museum of Utah.
WE ARE REQUESTING A $4.00 DONATION FROM EACH STUDENT TO HELP PAY FOR THE BUS. THE DONATION SHOULD BE PAID THROUGH THE FRONT OFFICE.
PERMISSION
I give my permission to have my child, identified below, participate in this event. I understand and agree that, for security and safety purposes, Ascent Academies of Utah will make its best efforts to ensure the safety and health of my child to, from, and during the field trip. I also agreeto waive all claims against, and to release the school of all responsibility and liability for any and all harm or illness that may come to my child during these times. This waiver of liability also applies to the school's employees, and volunteers.
