WORKING TOGETHER FOR CHANGE

Dear Parents and Carers of West Lothian

We have been approached by a local service provider ACTIVE HEALTHCARE SERVICES who aim to expand their service provision to families across West Lothian with children and young people affected by Autism Spectrum Disorders, Leaning Disabilities and Difficulties, by means of weekly supported groups.

It is proposed these groups can commence fairly immediately following direction of what best suits the potential service users. They aim to develop each child and young persons’ independence skills by supporting them to engage and participate in a variety of activities from the early planning stages right through to the completion to an end product.

In order to assist them to develop this service WLPCA and ACTIVE HEALTHCARE SERVICES have together devised the following questionnaire for our parents and carers to complete and return to the link below. Completion of this short survey will indicate to ACTIVE HEALTHCARE whatthe desired support needs and outcomes currently being sought by the children, young people and their parents and carers within West Lothian.

Your answers will help them develop and shape their new services to BEST meet the needs of these service users across West Lothian.

The information collated from the survey will be analysed as it is received, and thus the closing date of 07th july may be changed.

Supported Activity Groups- Survey

  1. What is your child or young persons diagnosis (es) and specific level of needs?
  1. What causes your child to feel especially good about herself or himself? This might include favourite activities and talents that they would prefer to engage in.
  1. Are there areas of your child’s development skills which you have specific concerns? How would you like to see him or her improve these skills perhaps through some group work?
  1. Is there any other information you would like to share about your child? Perhaps they have a specific way of communicating which they respond especially well to?
  1. What days of the week would you like the groups to run?
  1. What time of the day would you like the groups to run ? (A typical group will run 2 or 3 hours)
