Learning Session 1

Program Description and Objectives

The Community Health Center Association of New York State, in partnership with the New York State Department of Health and Mental Hygiene is providing a systematic approach to healthcare quality improvement in which NY community health centers benefit from lessons learned about improving access to carefor cardiovascular disease, and managing patient flow more efficiently.This is a 1 day learning session following the Health Disparities collaborative Breakthrough Series model tailored to NY community health center teams focusing on Orientation to the Care Model, Model for Improvement, and Cardiovascular Disease Clinical Measures (Decision Support).This educational event continues to bring quality improvement teams together to share lessons learned and gather new ideas.

Following the Learning Session participants will be able to:

  • Identify the six components of the Care Model and Name one example of a change concept for each component
  • Identify the required CVD tracking measures
  • Identify adequate clinical practice guidelines for treatment of patients with CVD
  • Identify two strategies to educate staff on proper blood pressure measuring techniques
  • Describe two methods to test support staff competency in the proper techniques of blood pressure measurement
  • Name at least one benefit to point of service lab testing
  • Write one draft of a standing order that will benefit patient care delivery
  • Identify one example of how technology can be used to care for patients with CVD
  • Describe two ways to identify patients to accommodate a planned care visit
  • List four adaptations of the care model that are beneficial in caring for CVD patients using a population-based approach
  • Identify at least two useful strategies to engage patients in self-management of their chronic disease
  • Define cultural sensitivity related to patient self-management
  • Identify at least two strategies for conducting a group visit led by Primary Care Provider
  • Identify at least 2 community agencies with which to partner
  • Define two strategies to reach out to existing patient education programs in the community
  • Draft a plan for next Action Period
  • List at least one new take-home idea for improving quality of care of CVD patients at their CHC

Resource: A Conversation Map is a series of images and metaphors on a 5 x 5 foot tabletop display. It serves as a facilitation tool for healthcare professionals to use to engage groups of patients in conversations around a healthcare topic such as diabetes, heart health or obesity.
Developed by the Canadian Diabetes Association and sponsored by the Merck Journey for Control program,this educational resource is dedicated to helping patients & healthcare professionals communicate about health information.

The clinical Regional Advisory Network, Region III, Inc. is an approved provider of continuing nursing education by the Delaware Nurses Association, an accredited approver by the AmericanNursesCredentialingCenter’s Commission on Accreditation. This event is co-provided with the Community Health Care Association of New YorkState and is acceptable for up to 6 nursing contact hours.
