Building sprinklers, or other fire suppression systems, are operational.
Cutting, welding, flame or spark-producing equipment is in good repair.
Precautions within 35 ft. of work:
Floors have been swept clean of combustibles; or combustible flooring has been wetted or covered.
All flammable and combustible materials have been removed if possible, or covered with fireproof curtains or metal shielding.
All sources of flammable vapors or combustible dusts have been eliminated.
All wall and floor opening have been covered.
Work on walls or ceilings:
Construction is noncombustible, including coverings and insulation.
Combustibles have been moved away from the other side of the wall or ceiling.

Work on enclosed equipment:
Equipment has been cleaned of all combustibles.
Containers have been purged of all flammable liquids.
Fire watch:
Fire watch will be provided during work period, including breaks, and for at least 30 minutes after completion of work.
Fire watch is supplied with proper type and size of extinguisher, or charged hose, and is aware of the location of the nearest fire alarm pull box.

CUTTING/WELDING/HOT WORK PERMIT
SCOPE: The cutting/Welding/Hot Work Permit is intended to be utilized by all employees, contractors and subcontractors who cut, weld or solder metal in Cincinnati Bell Telephone Company buildings. This does not include soldering work performed for central office telecommunication switching operations. It is intended as an instrument to notify building maintenance control centers when hot work is to be performed and to ensure fire prevention practices and procedures are followed.
DESCRIPTION: The Cutting/Welding. Hot Work Permit is to be included with all bids submitted by Real Estate Management and Network Maintenance Field Operations managers to contractors and subcontractors who are to cut, weld or solder metal in Cincinnati Bell Telephone Company buildings. The Permit is to be discussed by the building maintenance manager who is issuing the Permit in preconstruction meetings with contractors and subcontractors or with employees who are to perform hot work. Appropriate precautionary items that are required to be followed are to be indicated (checked) on the Permit by the building maintenance manager prior to the Permit being issued.
NOTIFICATION: Prior to commencing the hot work operation, the employee/contractor/subcontractor is to call Network Operations Center (NOC) on 566-3000 for hot work to be performed at all locations. The Network Operations Center (NOC), is to notify the CBT building maintenance manager who issued the Permit.
The Cutting/Welding/Hot Work (HR-403) should be retained at the worksite, then forwarded to Safety, 201 E 4th Street (102-M20), Cincinnati, Ohio, 45202, for record retention.
Firestopping - You must obtain approval and signature on the firestop application from Director – Corporate Real Estate & Facilities or a real estate department representative prior to opening a hole or creating a new hole.
