Do I Need to Submit this Report?

Organizations receiving funding through COCA’s Cultural Grant Program, administered on behalf of the City of Tallahassee and Leon County, are required to submit an interim report midway through the grant period. By doing so, applicants are confirming that each grant-funded project is proceeding and funds are being used as indicated in the original grant application (or approved via the amendment process).

When is It Due?

Interim reports must be received by COCA no later than 5PM on April 15th. If the 15th falls on a weekend or holiday, reports are due by 5PM the following business day. Once COCA has received and approved your interim report, the second half of your organization’s grant funds will be released. The release of grant funds is contingent upon the release of those funds by the City and the County.Reports can be submitted as early as April 1st.

Important:If you mail, fax or email a report, we strongly recommend that you call to confirm receipt by COCA. Late or missing reports result in the loss of technical points on the organization's next funding application (see grant guidelines for details).

How Do I Complete It?

Complete the certification page. And then complete one project section for EACH project that received funding during the grant period. Activities must have taken place between October 1 and March 31 of the grant period. No supplemental materials and no signature are required for this report.

The sum of funds shown on this reportmust be equal to or less than the grant funds received by your organization during the first grant disbursement (50% of your total award). If the project hasn’t yet taken place, you might show that a few dollars have been spent toward planning efforts, or perhaps no dollars have yet been spent at all. If the project has already been completed and some expenses were covered by other payment methods until receipt of the second grant disbursement, do not include those expenses on this report. Report only on the use of grant funds you already have in-hand.

Funding Recognition (NEW):

Please attach a pdf or other evidence of recognition of COCA funding from your publicity materials for your grant-funded program. (Please note: You are not required to submit proof of funding recognition until the final report.)

Funding Recognition: Grant recipients are required to include the following statement in all publicity materials (including programs, posters, flyers, and website) for any programming related to this grant.

“This program sponsored in part by the City of Tallahassee and Leon County through the Council on Culture & Arts.”

and/or to include City of Tallahassee, Leon County and COCA logos along with the above acknowledgement where space allows. All logos are available online at or by calling COCA at 850-224-2500.

Grantees which fail to acknowledge COCA, Leon County, and the City of Tallahassee in their marketing materials during the FY15 grant cycle will have 5 technical points deducted on the following year’s grant application.

Grantees will be required to submit copies of proof of grant acknowledgement at the time of the final report.
