2014 JA Nationwide, Inc Scholarship Application
JA Nationwide is the non-asset 3rd party logistics division of JA Frate Trucking located in Crystal Lake, IL. JA has been in business for over 40 years and a proud supporter our local high school graduates.
Please complete the following application questions by typing your responses on a separate sheet of paper and returning it to your respective high school guidance counselor no later than April 12, 2013. JA Nationwide will award a $1,000.00 scholarship to one graduating senior from a D155 high school at Awards Night of your respective high school.
1. Provide your name, address, email address and cell phone information on your application.
2. Attach a copy of your most recent transcript (Your GPA must be higher than 2.5).
3. What trade school, college or university will you be attending next year? (Attach a copy of your acceptance letter, if available).
4. Attach a written recommendation from a teacher or other faculty member as to your overall character and why you should receive this scholarship.
5. In 250 words or less, double-spaced, describe how your local community has impacted your life and/or you as a person.
6. Please list any extracurricular activities you have participated in during high school.
Staff from JA Nationwide will review all applications and will select the final three candidates for an in person interview at JA’s office in Crystal Lake. The winning applicant will be selected after these interviews.
Return the completed application to your respective high school guidance counselor no later than Wednesday, April 23, 2014 at the end of school day. Late applications will not be accepted.
