Introduction:

Complete thetemplate for the grade level and subject that you taught as follows:

  • Health/PE, Music, Art, Drama, Technology, Business, etc. - One grade level (minimum of 25 students; no more than 100 students)
  • K-8 Specialty Areas (Gifted, EIP, Special Education, etc.) –One grade level (minimum of 10 students)

Looking ahead:

In Step 3 of this application, you will be providing scores/ratings for Pre-Assessment, formative assessments, and Post- assessment for the 2012-2013 school year.

In Step 4, you will submit blank copies of the pre-assessment, one formative assessment and the post-assessment.

In Step 5, you will select one student from your roster in Step 3 and discussed this student’s performance related to the formative assessment that you explained in Step 4. You will provide a copy of the student’s performance results/work sample for this formative assessment. This document may be scanned if necessary.

Explanation of Data Submitted – Step 2
All of the following questions must be thoroughly answered on this template to explain
the data that you will be submitting on the student roster in Step 3.

Based on the results of the post-assessment, summarize the learning reflectedon the student roster in Step 3 by discussing the group’s progress toward mastery of the curriculum standards. Specifically, address the growth from the pre-assessment to the post-assessmentand the significance of this growth relative to school, district, state, and /or national results and expectations.
