This is a binding contract for Creative Kidz. Please read and initial before signing.
Full day child care is available from 7am to 6pm.
We offer morning and afternoon snacks.
You need to bring a lunch for your child.
Creative Kidzis a year round preschool and daycare
The AM program runs 9:00am to 12 noon. Your child can be dropped off as early as 8:00 and picked up between 11:45 and 12:00.
We know that you could use that extra hour.
Registration Fee: There is an annual registration fee of $45.00 payable at the time of sign up. $45.00 for 1 child or $75.00 for 2 kids. (Each year in February there will be an Annual registration fee of $25) ______
Payments: Your tuition is due on the 1st of each month.______
Late Fees: A 10% late fee will be added to any account not received by the 3rd of each month.______
Returned Checks: A $25.00 fee will be charged for any returned check, you may also be charged any bank fees.______
Withdraw Policy: 30 days notice of intent to withdraw Creative Kidz Program required. You are responsible for 30 days tuition from the notice date.______
Refunds: The registration fee is not refundable for any reason.______
Vacation: Your child’s tuition remains the same every month regardless of when he/she goes on vacation.______
Arrival: (daycare parents) If you have not arrived with your child by 10am Creative Kidz will not accept your child for the day. It can be very disruptive and we may have sent extra staff home by this time. If you need to come after 10 am please make special arrangements with AJ or Christina.______
Absences: You are expected to pay for all the days that your child is absent from group caredue to illness, snow days, holidays or personal reasons. You pay for the available spot. Due to scheduling we are unable to trade for another day.______
Snow days:If Newberg Schools are closed due to weather conditions, so will Creative Kidz. (Please watch your local news) Please call the Newberg School Closure Hot Line 503-554-5001 for closure information.
Illness and Sick Days: In order to control communicable illness in children and our staff, sick kids with fevers, diarrhea, severe colds, or any other contagious illness, may not be brought to group care.They WILL be sent home if they show signs of illness.
To follow the state guide lines ALL children must be free of fever for 24 hours prior to returning to school. Children who become ill with any of these symptoms will be sent home.______
Snacks for the class: Please refrain from bringing home made foods to share with the class. If you would like to bring snacks for a special event, please make sure it’s nut free and store bought.
Immunizations: In the interest of your Childs health and that of all the other children, you must submit your Childs current immunizations records prior to enrollment. These records must be updated in accordance with the state laws.______
Sunscreen: I authorize Creative Kidz Preschool to apply sunscreen on my child prior to going outside.______
Playtime: We have an outdoor play area for the children to play outside. Wedo have a rope with handles on it to secure the children’s safety. We do go to the park area down the street. During Park time all kids use rope with handles to get back and forth as well as a Teacher must have a cell phone on them with parent contact information in case of an emergency. ______
Diapers and Potty training: You will provide diapers, wipes and an extra change of clothes for your child.______

Full Time Childcare (preschool included)
Creative Kidz offers full time childcare. We are open Monday-Friday from 7am to 6pm.
We Charge a flat rate of $775.00 a month for 5 days a week. Individual days are available for a rate of $37.00 per day.
