Senior Support Worker

Why / Main Purpose of the Job:
  • Coordinate packages of care and support to individuals, delivering a service that promotes and supports choice, empowers individuals, helps people to take control of their own support and care and increases their independence and self determination.
  • To put the service user at the heart of your work, promoting each individual’s needs for independence, well being and dignity.

What / Key Accountabilities:
  • Work as a key worker for service user(s), working with an individual’s circle of support – such as care management, relatives, professionals and others in order to deliver the most appropriate service, ensuring that holistic planned programmes of support are coordinated with other agencies in the community
  • Agree, monitor and deliver real outcomes for individuals – actively working with others to develop good outcomes for service users
  • Develop and manage services delivered to a number of individuals, ensuring rotas are covered, there are correct staffing levels and that a truly personalised service is delivered.
  • Monitor, supervise, induct and appraise Support Worker level 1s and volunteers and coordinate the care and support they deliver
  • Manage all service user documentation, including Support Plans, Personal Care Plans and / or Health Action Plans
  • Be an able practioner in the delivery of support services, sharing skills, knowledge and expertise, role modelling good practice in the delivery of support
  • Develop proactive risk assessments that enable people to get the most from their lives.
  • Work to promote the rights of vulnerable people, providing a high quality service that incorporates the values of the organisation, working within our policies and procedures.
  • Take responsibility for and proactively deal with incidents, accidents and safeguarding issues
  • Encourage service users to be involved in the practical day to day running of their own home, providing assistance and life skills training that is appropriate to their abilities and which seeks to maximise their level of independence.
  • Support service users in developing and maintaining interesting and enjoyable activities within the community (social, educational, and employment opportunities). Assist service users with planning holidays and accompany them where necessary
  • Respect the confidentiality of the service user and the organisation at all times.
  • Maintain essential records in compliance with organisational policies and procedures, and where appropriate.
  • Support service users to maintain and exercise their rights as a citizen of the UK, ensuring that they receive the maximum level of welfare, and other benefits to which they are entitled.
  • Support service users to manage their personal finances and to understand and maintain their tenancy.

Requirements
Good knowledge of risk assessment and support planning
Good level of English language, literacy and numeracy skills
NVQ level 2 or 3 in care is desirable
Experience of providing high quality support to vulnerable people and of supporting a vulnerable person within the community
Use of IT Skills (Word, Excel Outlook) to prepare and monitor recording information and reports
Good knowledge of and experience of working within SP and / or CQC standards, the Valuing People framework and the in control agenda
