Comments:
Any person using equipment supplied with electrical power is at risk if that equipment is incorrectly or inadequately maintained or has not undergone periodic examination.
Installation, service and maintenance personnel are especially at risk if the equipment they are testing or repairing is not correctly isolated from all sources of electrical power.
Other persons not directly involved with the installation, test, or repair may be placed in danger if the correct procedure for isolation is not carried out.
Note:
Additional specific assessments may be required for the following activities (this is not an exhaustive list)
·  Work on or near "Live" equipment
·  Electric welding adjacent to water
·  Working near overhead power lines
·  Provision of electrical services during the installation phase of a contract
Hazards
Electrical hazards such as
1.  Electrocution (shock)
2.  Burns
3.  Fire and explosion
are all associated with the following:
·  Any fixed installation
·  Electrically powered tools, plant and equipment
·  Portable electrical appliances
·  Leads, connectors and other components of electrical tools and appliances

Control Measures (existing or planned)

Control / Details
Instruction
Information and
Training / 1.  Report faulty or damaged electrical equipment; do not attempt to use it.
2.  Do not repair damaged equipment or even fit plugs, unless you are authorised to do so.
3.  Never use or repair electrical equipment in damp surroundings or where flammable vapours may be present unless you know that it is suitable for that purpose.
4.  All Star Refrigeration employees will receive Tool Box talk No 17 (working with electricity)
5.  Any Star personnel required to operate electrically powered equipment will be given instruction and practical training in the use of such equipment.
6.  Do not use electrical equipment brought from home at work until it has been inspected and/or tested.
Supervision: / The Production Manager and Branch Managers will ensure that:
1.  Any electrically powered equipment in use by Star's employees is of an approved type, meets the requirements of relevant legislation and standards, and is used, tested and maintained to a safe standard.
2.  The fixed systems within all Star's premises will be installed to appropriate standards and legislative requirements, and will be maintained in a safe condition by testing at intervals not exceeding five years or at shorter intervals set by the competent person conducting the periodic tests.

Control Measures Continued

Control / Details
Supervision continued: / 3.  All portable and transportable equipment is maintained by testing and inspection at intervals detailed in SG.010
4.  All portable and transportable equipment will be 110 volt or if in excess of this will be supplied via a residual current device (RCD).
5.  Maintenance and testing of electrical plant and portable equipment is carried out by employees or contractors who have been trained and are competent
6.  All power supplies will be fully isolated and locked in the "off" position whilst work is in progress on the circuit or system.
7.  Suitable personal protection equipment is provided.
8.  Any defective equipment is withdrawn from use immediately the problem is discovered and is prevented from any possible inadvertent use until it has been repaired and tested.
9.  All persons required to install, maintain or test electrical equipment, whether employees or contractors' staff, have received training to ensure they have the competence to carry out the work in accordance with current legislative requirements and standards.
10.  PPE and safety equipment (barriers, signs, etc) is used when required to provide the requisite degree of protection to those working on electrical equipment and systems, and others operating in the vicinity of that work and who may be affected by that work.
11.  All personnel receive appropriate training in the safe use of electrical equipment
12.  Records of training are maintained
Environment; / 1.  Ensure that there is adequate clear space for access into control panels and that the floor is kept clear of trip hazards
Emergency action/procedures / 1.  Switch off and isolate supply
2.  Remove injured person(s) from danger area (if without risk)
3.  Render first aid
4.  Call emergency services

Risk Evaluation of Identified Hazards

Likelihood / Highly unlikely = 1 / Unlikely = 2 / Likely = 3
Severity / Slightly harmful = 1 / Harmful = 2 / Extremely harmful = 3
(Likelihood x Severity = Risk level) 1 = Trivial, 2 = Tolerable, 3-4 = Moderate, 6 = Substantial, 9 = Intolerable
Identified Hazards (base scores on existing or planned control measures) / Triv' / Tol' / Mod' / Sub' / Int'
1.  Electrocution (shock) 1x3
2.  Burns 1x2
3.  Fire and explosion 1x3 / 2 / 3
3

Note: Moderate scores; Thought should be given to reducing risk, but the cost and time requirements of prevention

Should be carefully considered and limited.

Substantial: Work should not be started until the risk has been reduced, for work in progress, urgent

action is required. Considerable resources may have to be allocated.

Intolerable: If unlimited resources cannot reduce risk, work will be prohibited.

Is risk adequately controlled using the above control measures Y/N / YES / If "NO" State further action required
State further action required:

Note : This risk assessment is only valid when all control measures are in place before the work or activity commences and will be reviewed 30/04/2016 or earlier if the nature of the work changes or an accident occurs.
