Contractor Requirements for Gulfstream Site Access

Purpose / The purpose of this document is to clarify the requirements for approval of Contractor firms for work on The Dow Chemical Company, Freeport Site, as part of the Gulfstream projects and to outline basic training requirements for individual contractor employees that will be working on the site.
Scope / This policy applies to all Contractor companies coming on to The Dow Chemical Company, Freeport Site. Other Dow sites will have different requirements.

Policy / All Contractor Companies seeking access to the Dow Chemical Company, Freeport for the Gulfstream projects must meet these requirements.

Contractor Requirements for Gulfstream Site Access

Company Approval for Working on Site / Each company must complete the Pre-Qualification/Approval Process before being given access to the construction site. Sub-contractor companies must be pre-qualified on their own and cannot enter Texas Operations under the umbrella of the company requesting their services.
Each Dow Chemical work site, Pre-Qualifies/Approves contractor companies independent of the other sites. A contractor company approved to work at one site is not approved to work at all other sites.
Companies working for the Gulfstream projects will be pre-qualified through the designated EPC(m) and will be approved by Dow representatives.
Documentation Requirements:
  • Pre-Qualification form submittedtoEPC(m), reviewed and approved by Dow Gulfstream EH&S.
  • Substance Abuse Program submittedtoEPC(m), reviewed and approved by Dow Gulfstream EH&S.
  • Hold Harmless agreement signed and approved by GulfstreamPurchasing or designee
  • Insurance requirements have been met and approved by DowCorporate Risk Management
  • P.O. or Requisition has been issued by GulfstreamPurchasing or EPC(m)

** Note: These requirements must be met either on a local or a global basis before approval will be granted.**

Contractor Requirements for Gulfstream Site Access, Continued

Pre-Access Meeting / Once the Pre-qualification/Approval Process has been completed and the company has been approved to work at the site, GulfstreamEH&S or designee meets with the contractor company management and safety to complete the Pre-Access Meeting.
The purpose of the Pre-Access meeting is to communicate the EH&S requirements and expectations for working at The Dow Chemical Company Gulfstream construction sites to the contractor company management.
Training Requirements for Site Access / The Contractor company must ensure all of their personnel complete the required EH&S training before beginning work.
Minimum Training Required for Issuing an Entry Badge:
  • Basic + or Basic + Refresher
  • Contractor Industrial Worker
  • Gulfstream Orientation
Additional training may be required dependent on the work the contractor will be performing. It is the Contractor company’s responsibility to contact the Dow construction manager to get the full job scope and understand what training is required.
Review the “GulfstreamContractor Training Policy”
Requirements for Driving Access / All requirements for the entry badge must be met, plus the individual must possess a Valid Driver’s License.
Individuals that will be driving on the plant site must successfully complete Driver training.
Review the “GulfstreamContractor Personal Car Pass”.
Requirements for Issuing anEntry Badge / Documents required to be issued a site entry badge:
  • Gulfstream Hire Slip
  • Training Card
  • Proof of identity

Pre-Job Requirement to Complete EH&S Activity Plan / Prior to beginning work on Site, All Contractors Must:
  • Develop an HSEactivity plan.
  • Communicate it to all contractor employees that will be working on the site
  • Identify by name, phone number, and cell phone number those individuals that are responsible for Environmental, Health, and Safety concerns.

EH&S Activity Plan Elements / The elements of the HSE Activity Plan include:

1 / Identify minimum requirements for the use of the Safety Task Analysis Card (STAC) and when training for crew leaders and foremen will take place.

2 / Outline specific personal protective equipment requirements for the job.
Review “GulfstreamContractor PPE Policy”
3 / Identify any specialized training that your employee will have
completed previous to beginning the job.Identify the method
used by your company to document required training
Review “GulfstreamContractor Training Policy”
4 / Identify the medical provider your company will use in the event of an accident.
5 / Specify the method that will be used to capture and report hours worked each month.
Review “Gulfstream Monthly Reporting Requirements”
6 / Provide a list of the names of At-Risk personnel that will be on the construction site and who their experienced buddy will be.
7 / Provide the foreman to crew ratio and the triggers for making ratio adjustments.
8 / Obtain a copy of the Gulfstream Administration Manual.
