Applications are invited for the post of Consultant Physician in Pulmonary Vascular and Respiratory Medicine. This post is based at Gartnavel General Hospital (GGH), Western Infirmary and the Golden Jubilee National Hospital (GJNH), with input into Pulmonary Vascular Disease (PH), Respiratory Medicine and General Medicine. The successful applicant would become the third consultant in the Scottish Pulmonary Vascular Unit (SPVU). [This unit has an international reputation for clinical care in pulmonary hypertension and basic and applied research.] The SPVU is funded by the National Services Division to care for all the patients in Scotland with PH. The Unit is part of the Cardiovascular and Medical Sciences Institute of the University of Glasgow and attracts funding from the Wellcome Trust, the BHF and the EU. One of the primary aims of this post is to expand the research output of the unit and he or she will have protected time to fulfill this objective for the first three years of the appointment. The respiratory unit at GGH (40 beds) has 7 other consultants with major subspecialty interests in asthma, allergy, sleep and ventilation, and cystic fibrosis. The successful candidate will join a fully integrated respiratory and general medical clinical service which maintains strong academic and teaching links with the University of Glasgow. The University will be asked to award an Honorary Clinical Senior Lectureship to the successful applicant.

Applicants must have full GMC registration, a licence to practice and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher specialist training leading to CCT or eligibility for specialist registration (CESR) or be within 6 months of confirmed entry from the date of interview. Non-UK applicants must demonstrate equivalent training.

Title of post: CONSULTANT PHYSICIAN WITH AN INTEREST IN RESPIRATORY MEDICINE AND PULMONARY VASCULAR DISEASE

Introduction

This post is a new post for a Consultant Physician with a special interest in Respiratory Medicine and Pulmonary Vascular Disease. In addition to joining the large team within Respiratory Medicine, the appointee will be one of three Consultants responsible for Scottish Pulmonary Vascular Service (SPVU) – funded through the National Service Division of NHS Scotland, providing care to patients from all over Scotland.

The appointee will work between Greater Glasgow and Clyde Health Board and the Golden Jubilee National Hospital in Clydebank, where the appointee will work predominantly within the SPVU.

The post is a joint appointment by the two Health Boards with GG&C Board being the main employer. You will be accountable to the Medical Director of the GG&C who will also be the 'Responsible Officer' when revalidation is enacted.

Both Boards are committed to developing a high quality service for patients requiring specialist services and work towards providing a world class reputation. The job description is in two parts to represent the content of each Health Board, firstly, that of the main employer.

GLASGOW

Glasgow and the immediate surroundings have a population of around 580,000. It is the largest city and the commercial capital of Scotland. The city has a vibrant cultural life, with municipal art galleries and museums, first class sports and leisure facilities, a wide range of theatres and restaurants, excellent shopping and is only 45 miles from Edinburgh.

Greater Glasgow and Clyde

Greater Glasgow and Clyde is the largest Health Board in Scotland and consists of eleven acute locations. The Health Board has close links with the University of Glasgow and makes a significant contribution to teaching.

The WIG/GGH complex is one of the main teaching hospital complexes of the University of Glasgow. It provides the Accident & Emergency Service for the West of Glasgow. It provides all major specialties including, Intensive Care, Coronary Care, Renal Transplantation and Dialysis, Beatson Oncology Centre including Haemato-oncology. The respiratory unit provides a comprehensive service for the diagnosis and management of all types of respiratory disease. The total population served is over 265,000. Patients are also referred for specialised respiratory investigation and treatment from other Health Boards.

General Respiratory Unit, Gartnavel General Hospital

Inpatients:

The unit receives all types of respiratory cases continuously. There are approximately 2,000 discharges per year. In addition to all the techniques and skills expected in any teaching hospital respiratory unit, we have special expertise in the following areas:

Cystic Fibrosis (Dr S Bicknell, Dr E Ross, Dr G MacGregor) West of Scotland Centre

Difficult Asthma [Dr M Shepherd, Dr N Lee]: The unit has particular experience in the investigation and management of cases of difficult asthma

Sleep related disorders of breathing [Dr S Banham]: A service to investigate and treat patients with disorders of breathing during sleep was started approximately 10 years ago and the Unit has seen a steady rise in referrals. Approximately 400 in-patients are investigated annually and also 400 patients at home.

Domiciliary Ventilatory Support [Dr S Banham]: Particular expertise in assessment and monitoring of neuromuscular conditions leading to respiratory failure.

Outpatients:

General Respiratory cases are seen at Gartnavel. The respiratory unit is responsible for all aspects of the care of acute and non-acute patients referred to the respiratory outpatient department. Over 1,500 new patients and over 9,000 return patients are seen each year. Respiratory Clinics are held at Gartnavel General Hospital. There are specialised clinics dealing with specific areas: Cystic fibrosis, Difficult asthma, Sleep related disorders. The unit has expertise in the immunotherapy of allergic diseases such as bee or wasp venom reactions.

Respiratory Liaison Service:

TB nurse specialist (n=1): undertakes TB contact tracing, BCG vaccination, supervision of DOTs and domiciliary assessment. There are between 45 to 50 cases of pulmonary tuberculosis notified each year from the West Glasgow Area.

Respiratory liaison nurses (n=3): run the Asthma & COPD service including a Rapid Assessment Unit, supported discharge service and domiciliary service for patients with severe COPD (mainly on LTOT).

Lung cancer liaison nurse (n=1): provides a lung cancer support service. Over 200 patients with confirmed lung cancer are treated annually and histological diagnosis obtained in 89% of patients

Cystic fibrosis nurse specialists (n=2): provide a service to the West of Scotland Adult Cystic Fibrosis Centre.

Pulmonary Vascular liaison nurses (n=2): provide a service to the Scottish Pulmonary Vascular Unit.

Bronchoscopy:

Four routine bronchoscopy sessions are undertaken each week with emergencies being accommodated at other times. The number of bronchoscopies (fibreoptic bronchoscopy/transbronchial biopsies) averages around 500 per year. The type of bronchoscopic procedures undertaken by the unit are as follows: fibreoptic bronchoscopy, transbronchial biopsy, EBUS guided transbronchial needle aspiration, bronchoalveolar lavage, laser therapy, photodynamic therapy, endobronchial electrocautery, bronchial stenting (in collaboration with radiology) and thermoplasty. The unit also performs medical thoracoscopy.

Respiratory Lung Function Laboratory:

The unit provides a specialised lung function service with laboratories situated at Gartnavel General Hospital, Golden Jubilee National Hospital, and the Western Infirmary. The laboratory is administered by Dr Martin Johnson.

The types of test undertaken include measurement of dynamic and static lung volumes, transfer factor and more specialised tests such as bronchoprovocation, exhaled NO, field exercise tests, cardiopulmonary exercise testing, flight assessment, respiratory muscle strength assessment and sleep assessment.

Educational programme:

The Respiratory Unit runs a weekly education programme including Seminars on Respiratory Medicine; General Respiratory Meeting: Multidisciplinary Lung Cancer Meeting; Radiology Meeting; Research Meetings.

Acute Medicine, Western Infirmary

Acute medical receiving takes place within a medical admissions unit in the Western Infirmary. Under the present system the duty consultant physician conducts a ward round in the admissions unit at 6pm and at 8am the following morning in collaboration with a continuity consultant. There are currently around 40 acute medical admissions per 24 hours. The post involves 8 weeks per year of cover of a medical ward at the WIG, 2 weeks of continuity cover over 12 months and 10 days of acute medical receiving cover.

Golden Jubilee National Hospital - National Waiting Times Centre Health Board
Introduction
The Golden Jubilee National Hospital (GJNH), which is part of NHS Scotland, has 300 beds overall with all wards having single or two bedded rooms with ensuite facilities. The estate is one of a high specification with a four star hotel and conference centre as an integral unit alongside the hospital. This has provided a very pleasing and attractive working environment for staff and a desirable patient experience. The main clinical services provided are heart and lung, elective major orthopaedics, general surgery, plastic surgery, diagnostic imaging and endoscopy services.
The GJNH is a state of the art tertiary referral centre on the banks of the River Clyde adjacent to the Erskine Bridge, in close proximity to Glasgow International Airport and within 30 minutes of the centre of Glasgow by road and rail links. A direct overnight sleeper rail service to Euston, London is available at the local station 5 minutes from the hospital. It is effectively situated west of Glasgow City and is minutes away from the countryside of the West of Scotland and Loch Lomond.
GJNH is managed by a special Health Board called the National Waiting Times Centre Board on account of its original function to undertake large amount of elective surgical and diagnostic imaging when it was first formed in 2002. It undertakes work for all territorial health boards in Scotland particularly in orthopaedics, general surgery, plastic surgery and imaging. Its original cardiothoracic surgical facility became the focus for centralising the West of Scotland and certain national heart and lung services. It continues to undertake large numbers of elective surgery.
The West of Scotland Heart and Lung Centre
The Centre was created in March 2008 bringing interventional cardiology and specialist surgical heart and lung services previously provided by three different units in the West of Scotland onto the one site under one management team. The interventional cardiology service which includes primary PCI is among the busiest in the UK. The Centre provides one of the primary PCI services in the UK and also the regional electrophysiology service for the West of Scotland. In addition, it is the centre for the Scottish Adult Congenital Cardiac Service (SACCS), the Scottish National Advanced Heart Failure Service (SNAHFS) and the Scottish Pulmonary Vascular Unit (SPVU). With this comprehensive range of specialist cardiopulmonary services for a catchment population of 2.2 million, the GJNH is one of the largest heart and lung centres in Europe.

The NHS National Waiting Times Centre Board is one of eight Special Health Boards of Scotland. It reports directly to the Scottish Government. The hospital has recently undergone a process of organisational change, restructuring of services into two divisions: Surgical Specialties and Regional and National Medicine.

National Waiting Times Centre Board Reporting Structure

The Scottish Pulmonary Vascular Unit (SPVU) at The Golden Jubilee National Hospital
The applicant will join the SPVU; the unit was founded in 1990 and was designated by the National Services Division (NSD) in 1998 to provide the services for all of the patients wilt Pulmonary Vascular Disease in Scotland. The unit is based at the Golden Jubilee National Hospital (GJNH), where there are facilities for Echocardiography; Cardiac Catheterisation; MRI; beds; Nursing and Medical support. Emergency admissions for these patients are to the Respiratory Unit at Gartnavel General Hospital.
The SPVU has also a strong research base with a basic science laboratory at the Western Infirmary studying Pulmonary Vascular responses to Hypoxia and Clinical Team at GJNH studying Pulmonary Haemodynamics and MRI. The successful applicant will have protected time for the first 3 years of the post to develop their research interests and obtain grant funding.
Patients with pulmonary hypertension are referred from all over Scotland; they are admitted to beds in the NSD pod at GJNH where they undergo a series of investigations including cardiac catheterisation. This is followed by the weekly Multidisciplinary Meeting attended by all SPVU clinical staff as well as Consultant Cardiologist John Payne and Consultant Radiologist Michael Sproule.
Pulmonary Vascular outpatients are also seen at the GJNH.
