Applications are invited for 2 Consultant posts which will be based at Queen Elizabeth University Hospital (QEUH). The successful candidates will join a team of 12 established consultants who provide care for patients referred to the Immediate Assessment Unit (IAU) at QEUH. In addition, the post holders will have commitments to work in the Medical High Dependency (mHDU) Unit at QEUH as well as delivering a medical consult service to the Beatson Oncology Unit inpatients.

Applicants must have full GMC registration and a licence to practise. Those trained in the UK should have evidence of higher specialist training leading to CCT in Acute Medicine or eligibility for specialist registration (CESR) or be within 6 months of confirmed entry from date of Interview. Non UK applicants must demonstrate equivalent training.

QueenElizabethUniversityHospital, NHS Greater Glasgow and Clyde

The QueenElizabethUniversityHospital is a new purpose built hospital that opened its doors in May 2015. The hospital has 1109 beds and provides medical care to a population of around 700,000 people in west and south Glasgow. The hospital provides the full range of medical and surgical services and many tertiary specialties including neurology, ENT and renal medicine.

Development of the new QEUH has allowed inpatient and acute medical care from the Western Infirmary, Victoria Hospital and Southern General to be consolidated to a single site. The presence of all the major specialities and the opportunities that this provides makes the QEUH an exciting and stimulating environment in which to work. As examples, Respiratory Medicine hosts cystic fibrosis and pulmonary hypertension tertiary care teams, Gastroenterology has inflammatory bowel disease and hepatology subspecialty teams, the Infectious Diseases unit is a regional service for tropical diseases and has a major HIV service, Rheumatology has a Professorial Unit with major interests in inflammatory arthritidies and connective tissue disease, and the diabetes and endocrine unit provide all aspects of subspecialty care in diabetes, as well as providing regional genetics, cancer and neurosurgical endocrine services. The QEUH also has one of largest critical care areas in the UK, with CCU, ITU and medical HDU areas.

Description of the Department

The Acute Assessment Unit of the Queen Elizabeth University Hospital has 118 beds including an Immediate Assessment Unit (28 beds), Medical and Surgical speciality clusters (90 beds) and an Ambulatory Emergency Care unit.

Immediate Assessment Unit

The Immediate Assessment Unit (IAU) has 28 beds and was planned to see circa 20 000 new GP referrals per annum. A consultant presence is maintained in the Assessment Unit currently between the hours of 0800 - 2000 hours Monday to Friday and 0800 - 1600 hours Saturday and Sunday.

The IAU provides a full 24 hour a day 7 day a week service for all medical and general surgical GP referred patients. All patients requiring admission from the IAU or the Emergency Department for medical or general surgical management are admitted directly to the 90 beds in the speciality clusters in the Assessment Unit.

The IAU is adjacent to the Emergency Department and radiology allowing early swift access to all investigations required for patients on their initial presentation. This process supports early decision making and management of patients for admission to or discharge from hospital. We have close working relationships with the Emergency Department to support patients’ presentations that may require Emergency Department level of support in resus.

Ambulatory Emergency Care Unit (AECU)

This unit is for the management of low acuity patients referred by their GP with medical conditions, with most patients being discharged the same day. Patients managed in the AECU benefit from timely assessment and diagnostics where necessary with the opportunity to refer patients into emergency ‘hot’ clinic slots and Day Ward services as required. Many patients seen in CDU will be managed using protocolised pathways for conditions including Chest pain, cellulitis, and headaches, with an expected ongoing expansion to support ambulatory care. The CDU is staffed by 2 GPs with an interest in Ambulatory care, as well as a team of Medical Nurse Practitioners.

Medical HDU, QEUH

This is a 9 bedded ward in the QEUH. Monday to Friday from 8am to 6pm there is Consultant cover provided by a rota of Intensive Care and Acute Medicine Consultants. Out of hours and at weekends the unit is covered by on call medical Physicians.

The Beatson Oncology Centre

This specialist cancer care centre is located next to Gartnavel General Hospital and provides inpatient and outpatient non-surgical cancer services.

The Job Itself

Title: Consultant Physician in Acute Medicine

The Acute Medicine component at QEUH will be made up of 4 PAs of clinical activity working shifts in the IAU. Consultant input to the care of patients in the AECU will also be expected. The successful candidate will be expected to work up to one late shift each week until 10pm in the IAU. The remaining shifts will end at 6pm or earlier during the week. Weekend cover in IAU is 8am to 4pm, on a 1 in 8 basis

Medical HDU – the job plan includes 1.25 PAs of time working in mHDU. This equates to one 8am to 6pm shift every 2nd week in mHDU

Beatson – each post holder will have approximately 2.5 PAs in their job plans dedicated to activity at the Beatson Cancer centre. The aim of this is to provide medical input to the care of inpatients at the Beatson centre on a consultative basis. There will be no ongoing commitment to these patients out with of these sessions, their inpatient care remains under the supervision of the Beatson teams.

Teaching

Undergraduates attend throughout their training. In the QEUH there is a commitment both to supervise students in clinics and bed-side teaching. There is postgraduate teaching for junior staff in the directorate and a weekly Grand Round.

Research

There are audit and research opportunities including the possibility of establishing research links with colleagues in the University of Glasgow.

Administration

Time for review of results and discharge letters (EDLs) for patients discharged from IAU and AECU will be included in the job plan

SPA time will be allocated within the job plan to support the post holders personal appraisal and revalidation work.

This job plan is negotiable and will be agreed between the successful applicant and the Clinical Director. NHS Greater Glasgow & Clyde initially allocates all full time consultants 10 PAs made up of 9 PAs in Direct Clinical Care (DCC) and one core Supporting Professional Activities (SPA) for CPD, audit, clinical governance, appraisal, revalidation, job planning, internal routine communication and management meetings. The precise allocation of SPA time and associate objectives will be agreed with the successful applicant and will be reviewed at annual job planning.

It is expected that 9 programmed activities per week will be devoted to direct clinical care, with 1 programmed activity allocated for supporting professional activities.

Out of hours cover for medical patients / IAU patients is delivered on a rotational basis, shared with other IAU and Medical Consultants. The successful candidate will be part of this rota and receive an appropriate availability supplement for this.

The Beatson Cancer Centre require 5 sessions of Medical Consults each week to their patients. This time will be shared between the 2 successful candidates for these posts, with each delivering 40% of this activity. The remaining 20% of this will be provided by existing IAU consultants. As this activity is to be delivered all year round, prospective cover will mean that around 2.5 PAs of job plans are required for this.

The conditions of service are those laid down and amended from time to time by the Hospital and Medical & Dental Whitley Council.
