CONSENT FORM FOR: headspace Cairns Swim, Surf and Splash Party

To be returned to School or headspace Cairns by: Thursday, 30th March 2017

As part of National Youth Week 2017 headspace Cairns are hosting a Swim, Surf and Splash Party on Sunday the 2nd of April 2017 at Tobruk Pool from 1.00pm – 5.00pm.

There will be a bus service available for young people wishing to attend.

Young people under the age of 18 will need to submit a completed parent/guardian permission form and return to your school office by Thursday 30th March 2016.
