  • If not already neutered, the dog/bitch must be speyed or neutered within 6months of re-homing.
  • The adopter is responsible for insuring the dog
  • Under no circumstances may an un-neutered dog/bitch be bred from
  • The dog must not be sold, given or passed on in any other manner to any other person. In the event of the adopter being unable to keep the dog for any reason, it must be returned to SHWA(UK) with all relevant paperwork and must not be re-homed in any other way without the express agreement of SHWA(UK).
  • The dog will be kept in appropriate conditions for its health and welfare and will be fed a suitable diet for its age and condition

I have read and understood the above conditions of adoption and would like to be considered as a potential adoptor for a SHWA(UK) Welfare Dog. I understand that this will involve a home-check and a reference from my veterinary surgeon.

Please Note – If the home check involves our assessor travelling more than 5 miles, the applicant will be expected to contribute towards the assessor’s transport costs at a rate of 15p per mile (up to 25 miles) and 30p per mile thereafter..
