POSITION SUMMARY:Under general supervision, is primarily responsible for compiling data forCollaborativesand Performance Improvement initiatives. Responsible for the maintenance of the Collaborative registry.Maintains all patient data in accordance with the Bureau of Primary Health Care recommended standards.
JOB-SPECIFIC COMPETENCIES/ESSENTIAL FUNCTIONS/DUTIES & RESPONSIBILITIES
Accurately abstracts pertinent information from the medical record needed for Collaboratives and Performance Improvement initiatives.Enterspatient’sclinical information into Collaborative Registry.
Assists with compilation and aggregation of data from all clinic sites for Collaboratives.
Preparesand submits monthly reports to the Health Disparities Midwest Cluster office.
Assists with preparation and submission of Quarterly Narrative reports to Health Disparities Midwest Cluster office and CHC Quality Assurance Committee.
Performs periodic chart audits.
Actively participates in Collaboratives via attendance at team meetings and conference calls.
Works with computer programs such as Microsoft Word, Access, Excel, Powerpoint and Health Pro .
Maintains graphs of Providers registry data.
Maintains Collaborative registrieswith current and accurate information at Rock Island, Moline and River Drive Clinic sites.
Performs Quarterly updates of Inactive data base as indicated by the Inactive protocol.
Update colloborative status in HealthPro of any new, inactive or deceased patient changes to the registry.
Generates reports in Health Pro and PECS as indicated.
Attends off site training/meetings as indicated.
Uses and understands basic medical record practices and procedures, and medical terminology.
Demonstrates excellent attention to detail

Experience:Two years working in a Medical Environment. Basic knowledge of Medical terminology, and computer applications such as: Microsoft Word, Excel and PowerPoint.
Hours of Work:8:00am-5:00pm Monday-Friday

Physical:Requires sitting and standing associated with a normal office environment. Manual dexterity using a computer keyboard.
