GRANTEE FINAL REPORT FORM

SPRING 2011 GRANT CYCLE

Please submit a Grantee Final Report at the end of the funded project or no later than July 1, 2012. Completed reports may be mailed to Community Foundation of Cedar County, P.O. Box 432, Lowden, IA 52255 or submitted electronically in Microsoft Word or Adobe PDF format by e-mail to . FURTHER GRANTS TO YOUR ORGANIZATION CANNOT BE MADEUNTIL A REPORT, EVEN IF IT IS NOT PAST DUE AT THE TIME OF THE APPLICATION DEADLINE, HAS BEEN SUBMITTED. If the amount of fundsrequestedwas not 100 percent funded, you are still expected to complete the project as proposed. This may require you to do additional fund raising. If you are not able to obtain the additional financing to complete the project as proposed, you will be expected to notify the Foundation and return the funds. Please do not downsize or change the project to the amount awarded or to reduce your expenses. Do not cash the grant check unless you are certain you will be able to complete the project.

The Final Report is a very important part of the Foundation’s grant making process. It enables the Foundation’s contributors and Board of Directors to evaluate the impact of individual grants and focus future funding on areas where the Foundation’s resources can make a difference.

We appreciate your time in preparing a Final Report. We would ask that the Report not exceed two pages and that you address as many of the following topics as are appropriate to your project. You may retype the questions to allow for more space and manipulation of material but please leave it in the format presented. The report must be TYPED. Handwritten reports will not be accepted.Also, please note that all reports MUST INCLUDE a comparison of budgeted expenses to actual expenses and copies of invoices paid. Thank you.

If the entire allocation has not been expended, arrangement should be made with the Foundation for its disposition. Thank you.
