The equipment description form is part of the documentation relating to measurements (i.e. the “Test Report”).

AP-3

EQUIPMENT DESCRIPTION FORM FOR TESTING

The application form shall be completed by the applicant and submitted to a test laboratory accredited in accordance with EN45001 and EN45002 or ISO guides 25 and 38 or a national standard conforming to EN45001 and EN45002 or ISO guides 25 and 38.

PLEASE READ THE FOLLOWING NOTES

Note (a)

More than one item of equipment may be required to be submitted for testing where an equipment needs to be modified to provide connections to facilitate testing, i.e. measurements requiring direct connections to be made, for example audio measurements. In such cases a second unmodified equipment may be required for radiated i.e. case or cabinet radiation or ERP (effective radiated power) measurements to be performed.

Full details of modifications are to be provided.

Note (b) (1)

Some ENs (and ETSs/IETSs) allow for variations in frequency range, maximum power, channel separation, etc., to reflect differing requirements. Manufacturers producing equipment conforming to these standards may wish to offer an equipment of the same basic design in several variants each with differing RF characteristics such as frequency range, maximum power, etc.

Manufacturers may also wish to offer a "family" of equipment with differing operational features of an optional nature e.g. selective calling. Guidance to applicants, relevant to the standard in question on the resting requirements for equipment with such features should be given at this point in the application form, e.g. reference to the standard or relevant ETSI Technical Report.

Each member of a family with a different optional operational feature shall carry a unique type designation.

( 1 ) To be used where relevant to the standard concerned. Should be adapted to the requirements of the concerned standard.

Note (c)

Accessories

It is expected that equipment is provided with appropriate connectors to facilitate the connection of test equipment by the test laboratory to the equipment under test (E.U.T.). This should enable:

a) access to the equipment RF power output (transmitter) and RF input (receiver);

b) operation of the equipment "press to talk'' transmitter operation;

c) access to the equipment modulator audio input;

d) access to the receiver audio output. If the equipment has more than one audio output then suitable connectors for these are also required. It is not permissible to access the receiver audio output in parallel with loudspeaker;

e) means of connecting the equipment to an external power supply.

Note (d)

When measurements are performed on a preproduction model, that model must be manufactured to the same production drawings and manufacturer's specification as the later production models.

Where this is not the case, either partial or full measurements are expected to be carried out on the final production equipment.

Note (e)

It is the manufacturer's responsibility to ensure that the equipment meets all the regulatory requirements for marking in the country where the equipment is expected to be placed on the market.

Note (f)

The title page of the Test Report shall include the following statement:

"

The documentation shall consist of two parts

Part one. Application Form

Part two. Test Report Form

"

Note (g)

If particular abbreviation(s) or acronym(s) which are not described in the standard (e.g. w.r.t. means which refer to ...) are used, a glossary of these abbreviation(s) or acronym(s) is to be included.

Note (h)

It is the applicant's responsibility to try and obtain information on the appropriate technical information.

However, it may occur that an applicant is not in a position to complete or to provide all items (e.g. test fixtures).

The box relating to the Applicant’s details has been changed for the purpose of this Application Form.
