Supplementary Application Deadline: FEBRUARY 26, 2016

Please note: A minimum GPA of 6.0 (B) or above is required

1)If you are applying to multiple institutionsincluding York, apply through the Ontario

University Application Centre(OUAC) at the following link:.

2)If you are applying onlyto York, apply using thefollowing York Admissions website,.This initial application is available in October/November each year and is dueFebruary 3, 2016, along with the application fee.

3)Once you apply through OUAC or York Admissions, you will receive an acknowledgement

Letter/emailwith your9-digits York Reference Number and gain access to 'MyFile' at:

Here you can access the online Supplementary Application Form within 72 hours

aftersubmitting your initial application. The acknowledgement letter will outline all documents

that are required for the Social Work Supplementary Application Form.

4)The Supplementary Application Form is to be comple48965ed online. Please note:Should you encounter

difficulties submitting online, the following admissions documents must besubmitted to the Office of Admissionsin the Bennett CentreNOT to the School of Social Work.

The information and documents required are:

Personal Statement(See Supplemental Application Form for your Personal Statement requirements)

Problem Statement(See Supplemental Application Form for your Problem Statement requirements)

Transcriptsfrom all post-secondary institutions attended

Resume

3 References. Reference contact information is to be submitted through ‘MyFile’. This

will automatically send an electronic form to your referee requesting a reference letter on

your behalf. Your referee should complete the request and send it electronically to

the York Admissions.

The Supplementary Application Form and documents are dueFebruary 26, 2016.

Applications will not be reviewed until all documents have been received.
