STUDENTHEALTHSERVICES

General

TheStudentHealthServicesProgramisfundedbytheStudentHealthFeeas establishedbyBoardPolicy5030.Intermittent,alternativefundingsourcesfromspecial projects,partnershipswithcommunityprovidersandagencies,grantsandprograms providesupplementalincomeforprogram/serviceenhancement.Studentsenrolledin anycreditcourseareprovidedserviceswithoutadditionalcost,withtheexceptionof minimalchargesapprovedbytheHealthServicesStaffinconsultationwiththeMedical Advisor/Physician and endorsement of the Health Advisory CommitteeTeam.

TheHealthServicesAdvisoryCommitteeTeam,chairedbytheCoordinator,has representationfromcontractedMedicalAdvisor,Faculty(COSTA),Classified(CSEA), Administration,studentandcommunityorganizations.CommitteeTeamfunctionsand responsibilitiesinclude:evaluationofcurrentservicesrelativetocollegeneeds,review ofhealthfeeincomeandexpenditures,development ofrecommendationsfor expansion,improvementandenhancementofprogramservicestotheCoordinatorand college administrators.

TheprimaryStudentHealthCenterwithitswidevarietyofprogramsandstaffis establishedatthemainVisaliacampus.LesserlevelsofserviceandscopeSimilar servicesare deliveredtoontheHanfordand Tularesatellitecampusesonaregularlyscheduledbasis,contingenton demonstratedneed,analysisofassessmentdata,andutilizationpatterns.Servicesare availableduringthetraditionalacademicyear,/fallandspringsemestersandportionsof the summer sessions., with daytimehours of operation including both day and evening times.

WithsStudenthealthfeefundsarerestrictedtothedeliveryofservicestoregisteredstudents only,.sServicesprovidedtoDistrictemployeesarelimitedtofirst-aid/emergencycare, healthscreeningservicesduringcampus-widehealthpromotionevents,andscheduled EmployeeWellnessactivitieswhenalternatefundingisavailable.Mandatedemployee Tuberculosis testing and otherhealth screenings/clearances, and designated employee- requiredHepatitisBvaccinationarefundedbytheDistrict.Fluvaccinationandother immunizations and services are charged topaid by the employee at time of service.

Staffing

TheStudentHealthServicesCoordinatormustfulfilltheminimumqualificationsfora HealthServicesprogramsupervisorasoutlinedinTitle5(CaliforniaRegisteredNurse License,Master’sDegreeinhealthrelatedfield–orequivalency,andPublicHealth Nurse Certificate.)

Permanentand contractednon-teachingfacultyandpermanentandpart-timeclassifiedstaffpositions areutilizedwithinthedepartmentappropriatetothelevelofserviceandfunding capabilities.

FederalWorkStudyandCal-WORKSstudentemployeesperformavarietyofoffice support activities including reception, clerical, and outreach duties.

ThePhysicianMedicalAdvisorfromthecommunityiscontractedhourlytoprovide medicaloversight,reviewandapprovalofprotocolsandprocedures,consultationto professionalnursingstaff,andrepresentationonHealthServicesAdvisoryCommitteeTeam and Program Review Team.

TheAPpart-time/Hourlyadjunct Licensed Marriage Family Therapist or Licensed Clinical Social WorkerProfessionalExpert(fromSocialScienceFaculty)provides supervision for LCSW and MFT practicum and intern students who deliver psychological counselingservices.

Requiredlicensureandcertificationsforallprofessionalstaffareverifieduponhireand monitored annually or as indicated by renewal date.

Clinical Care Services

EducationCodeSection76355authorizescommunitycollegestoofferhealth supervisionandservices,andtochargeallregisteredstudentsafeeforthoseservices (AB982).Title5,CaliforniaCodeofRegulations,establishedthetypesofhealth servicesaDistrictmayofferandspecifiedthetypesofexpensesforwhichthehealth services fees may be used.

ThemajorityofservicesprovidedbytheStudentHealthCenterareprovidedbyDistrict-employedCollegeCalifornia State Licensed RegisteredNurses.(Master’sandBachelor’sdegreeRegisteredNurse licensed/PublicHealthNursecertificated.)Apracticing/licensedphysicianfromthe communityservesasaMedicalAdvisorandconsultantonacontractualbasisatan agreedhourlyfee-for-servicerate.Contractswithcommunityproviders,i.e.,Planned ParenthoodandTulareCountyDepartmentofHealthandHumanServices,for expandedservicesmayincludetheassignmentofmid-level/NursePractitionerstaffand medicalassistants,mentalhealthpractitionersand/ormaster’slevelMarriageand FamilyTherapist(MFT)/LicensedClinicalSocialWorker(LCSW)internsunderthe supervision and employment of the contracting/District agency.Expanded services may include contracts and/or employment with community providers, i.e. mid-level nurse practitioners, medical assistants, and licensed intern/trainee mental health practitioners under the supervision of the contracting/district agencies.

Clinical Care Services include:

A.Assessment, intervention and referral for acute/episodic physical conditions Registered Nurses provide health care within their scope of practice and

licensureasestablishedbytheRegisteredNursePracticeActwhich may include assessment, intervention and referral for acute/episodic physical conditions.Standardized ProtocolsandProceduresaredevelopedinconsultationwiththeHealthServices Coordinator,MedicalAdvisorandAdvisoryCommitteeTeam.Includedarespecific parametersforClinicalLaboratoryImprovementAmendmentsCertificateof Waiver(CLIA)approvedlaboratorytesting,immunizations,over-theCounter medications and approved procedures.,Wwiththeexceptionofon-campusaccidentorinjurythatmaybecoveredby Workerman’sCompensationorstudentaccidentinsurance,;uninsuredstudents requiringservicesorsuppliesnotavailableunderprotocolareassistedinreferral toappropriatecommunityresourceswheretheyaresubjecttotheburdenofany associatedcost.Studentswithinsurancearereferredtotheirprivatephysicianor usualsourceofmedicalcarewhenneedisindicated.Ongoingclinicalcarefor chronic,long-termconditionsisnotprovided,however,periodicmonitoringof certainconditions(diabetes/glucosetesting,hypertension/bloodpressure monitoring/cholesterol-lipidscreening)forself-managementandmaintenanceis available between scheduled physician visits.

B.First Aid and Basic Emergency Care

District Police and/or local police agencies respond to all emergency calls on the three campuses. Students who can safely transport themselves to the Health Center will be assessed and provided first aid, basic emergency care, treatment and/or emergency transportation to a local healthcare facility if indicated. Administration is to be notified whenever a student is transported by ambulance. the student’s name is omitted in notification in compliance with HIPAA regulations.

StudentHealthServicesco-respondswithCampusPoliceformedical emergencieswhenlicensednursingpersonnelareavailableatthecampussite. Basiclifesupportandfirstaidareprovided,aswellasassessmentofpatient needforadditionalcare,includingemergencytransportasindicated.All employeesinStudentHealthServices,aswellas

Health Center nurses and DistrictCampusPolice,aretrained andcertifiedinCardio-PulmonaryResuscitation(CPR)andAutomatedExternal Defibrillator(AED)use.ProvidersemployedbyStudentHealthServiceswithother specializedemergencycarecertificationswillnotbelimitedfromusingtheirskills inemergencysituations,aslongasallmandatedandproceduralrequirements arefollowed.StudentHealthServiceslicensedstaffpersonnel mayprovidesconsultationto other staffemployeesregarding emergency proceduresat off-campus sites.If students are too injured or ill to safely transport themselves to the Health Center, emergency services are activated through a 911 call.

C.Health Appraisal

Healthscreeningsareprovidedbyqualifiedpersonnel,includingbloodpressure checks,basicvisionandhearingscreeningsandCLIAwaivedlaboratorytests.Health riskassessmentservicesareprovidedbyavarietyofprofessionals,with appropriate counseling and referrals andwhichmay include: administration of American CollegeHealthAssociationHealthRiskAssessmentSurvey,depression screening,glucose/diabetesscreening,bloodpressureand cholesterol/triglyceridescreening,disorderedeatingdisorderassessment,sexually transmitted diseaseinfection/HIV and pregnancy screening.

D.Communicable Disease Control

Surveillanceandscreeningforavarietyofcommunicable/infectiousdiseases, treatment,casemanagementsupportservices,immunizationsandother preventionservicesareprovidedbyHealthServiceslicensedstaffin collaborationwiththeTulareCountyDepartmentofHealthandHumanServices tocontrolpotentialoutbreaksofcommunicablediseasesonthecollegecampus. Mandatedreportingofalldesignatedcommunicablediseasestothelocalhealth jurisdiction is executed per protocol.

Staffnursesarestate-certifiedHIVTestCounselorsandprovidetestingfor studentsonrequestinaccordancewiththeTulareCountyHealthHuman Services Agency (TCHHSA) protocol.

Immunizationandtuberculosis(TB)skintestingareprovidedtostudentsand staffemployeesundercountyprotocol,basedonpublichealthandnationalcollegehealth standards.StudentHealthServicesprovidesassistancetoallstudentsin meetingmatriculationrequirementsthatfacilitateprogramaccess,i.e.,college transfer,RegisteredNursingandAlliedHealthprograms,ChildDevelopment, studentemployees,DdistrictemployeeTBclearanceandHepatitisB immunization (for designated/at risk employees only).

E.Reproductive Health Care

Sexuallytransmitteddisease(STD)andpregnancyriskassessment,counseling andtestingserviceshavebeenpreviouslyaddressed.EC(Emergency Contraception)isavailableonrequestfollowingconsultationwithlicensedclinical staff.In partnershipand contractual agreement with third-party agenciesa community provider, comprehensivereproductivehealthservicesincludeing:sexualhealth/risktaking assessmentandcounseling,sexually transmitted infections (STI)STDtestingandtreatment,pregnancy testing/counselingandreferral,screeningforcervicalandbreastcancer, contraceptivesuppliesandhealtheducationareprovidedforbothfemaleand male students two days per week.

Mental Health Care

Mental,emotionalandpersonalcounselingservicesareprovidedthroughattheHealth Centerinathroughsupervisedpsychologyinternship/traineestructuredprogram,withMaster’slevel and graduate psychology practicum and intern students.throughcontractual agreements with multiple private and state colleges and universities.Contractual agreements are made and implemented with qualified students and multiple private/state/university colleges to provide quality counseling services.Required on-site supervisionof student-counselorsisprovidedbyafullpart-time, Master prepared contracted, non-teachingfacultymemberwhoisaLicensed Clinical Social Worker.or a Licensed Marriage Family Therapist.

A.Crisis Management

Licensednursingstaffco-respondwithCampusDistrictPoliceare called first to respondtopsychological emergencyemergenciescallsoncampus,.assessthestudentneedforfurthercareand evaluationeitherattheHealthCenterorbyanoutsideagency,andmake appropriatearrangementsforemergencytransportorreferral.Assessment of student needs for further evaluation may be conducted at the Health Center or by an outside agency to make appropriate referrals or arrange emergency transport for additional care. Licensed therapists,Ccounseling practicum/interns(whenavailable)andstaffnursesprovidecrisisevaluation, interventionandreferralservicesforalldrop-incrisesduringopenhoursofclinic operation.

TheDistrict’sCrisisBehaviorInterventionTeam(BIT)includesrepresentationfromCampusDistrictPolice,DeanofStudentServices,HealthCenter Coordinator and a licensed mental health counselor.ServicesandSocialSciencefaculty andThe BITassistsintheinterdisciplinarycasemanagementofstudentsthatareincrisis and/or disrupting the educational environment due to behavioral problems.

B.Short-term Psychological/Personal Counseling

Short-termtherapy, consisting of six to eight sessions,isprovidedbypsychologythe counseling supervisor andpracticumstudentsandinterns/trainees (underpreviouslydescribedfacultysupervision)toCOSstudentsthroughself-referralorreferralbyHealthServicesorotherfaculty/staff.Studentmental healthassessmentmayincludebut not limited to:suicideideationandprevention,stress management,drug/alcoholintervention,disorderedeatingdisorderevaluationand interventionandortherapeutictreatmentofpsychologicaldifficultiesthatimpact academicperformanceandretention.Lengthoftreatmentis consideredaclinicaldecisionthatisbasedonthespecificneedsofeachstudentandhis/her progressovertime.Directcarefordiagnosedchronic/long-termconditions,i.e., schizophrenia,bipolardisorder,andpersonalitydisorder,isnotprovided, however,studentsareassistedwithreferraltoappropriatecommunityresources and supportive services as needed when on campus.

Incident Reportsand Insurance Forms

  1. Student Injuries

Incidentreportsofon-campusorcollegesponsoredactivityaccidentorinjuryare generatedthroughtheHealthCenterinconjunctionwithnursingassessmentwhen possibleorreviewofreportifgeneratedelsewhere.A finished report, (consisting of a HIPAA Individual Authorization and Accident Claim Verification) is sent to the Student and Athlete Insurance Network. Both forms along with the Incident Report and verification are forwarded to the Executive Administrative Assistant in Administrative Services.

  1. Employee or Student Worker Injuries

Employees injured on campus or student workers injured on the job must report injuries to Human Resources for the completion of appropriate Workers’ Compensation forms and referral to designated care provider for evaluation and treatment.Whenindicated,referralisinitiated andreportforwardedtoPayrollforWorkerCompensationor

  1. Intercollegiate Sports Injuries

Intercollegiate sports injuries are reported to the Athletic Department for evaluation, treatment, and completion of appropriate forms for student accident insurance processing.Athletics/Physical Educationforstudentaccidentinsuranceprocessing.

Documentationismaintainedin Health Center for seven (7) years.

Medical Records

MedicalRecords, in both physical and electronic formmaintained in an Electronic Health Records format, reflectaccurate and timely documentationofallclinicalencounterswithstudentsandstaffemployees.Recordsare maintained,inaccordancewithstateandfederalstatute,forseven(7)yearsfollowing thelastdateofservice.InaccordancewithmandatedHIPPAHIPAA(HealthInsurance ProtectionandPortabilityand AccountabilityAct)federalregulations,medicalrecordsinpart,orinfull,may bereleasedonlyuponwrittensubpoena,ortothestudentand/orhis/herdesignated outsidehealthagencies/providers/individualsuponwrittenauthorizationbyand consultation with the student. Students may be charged a fee to obtain copies of medical records.

Confidentiality/Mandated Reporting

AllcontactswithStudentHealthServicesarekeptconfidential,inaccordancewiththe above HIPPAHIPAAprotections, with the following exceptions,which are specified inbylaw:

1.When the student is determined to be a threat to self or others

2.Wheninformationisrevealedtoastaffmemberaboutanepisodeofchildor elder abuse

3.When diagnosis of a “reportable”/communicable disease” mandates official reporting to/notification of the local Public Health Authority.

Health Promotion and Education

StudentHealthServicesstaffintegrateshealthcounselingandeducationintoeach personalencounterwithstudentsandstaff,withtheconvictionthat“everyencounteris ateachableopportunity.”Collaborationwithacademicdepartmentsandfaculty frequentlyincludeclassroompresentationsonavarietyofhealthtopicsascurriculum infusion/integrationcomponents.Healthriskassessmentsurveysandannouncements of new services include classroom delivery.Monthly health events are scheduled in outdoor facilities or classrooms to provide physical/mental health education to promote student success through health promotion and disease prevention.

CollaborationwithTulareCountyDepartmentofHealthandHumanServicesprograms andstaffandotherprivate,publicandnon-profitcommunityagenciesfacilitates programmingofmultiplehealthpromotionandawarenesseventsthroughouttheyear including:regularlyscheduledbloodandbonemarrowdrives,healthfairs, alcohol/tobaccoandotherdrugpreventionevents,women’shealth,cancerand cardiovascularhealth,sexualhealthandrelationshipviolenceandassaultprevention awareness,HIV prevention and other relevant college health topics.

Abroadselectionofhealtheducationresources,includingpamphletsandbrochures, videos, books, periodicals and other media are available for student use.

Aninteractivewebpageismaintainedandupdatedtoreflectcurrentinformationand authenticityof resource links.

CollaborationwithCampusNewspaperstaffandreporters,and the District’sCampusPublicInformationOfficer, as well as use of theEventCalendar,ElectronicCampusMessagingSign,posters,banners,displaycasesandspecialeventhangingsigns,combinedwithstaffandstudentadvocacyare effective outreach and marketing strategies.

Teaching and Research

StudentHealthServicesprovidesclinicallearningandskillspracticeopportunitiesfor studentsintheRNProgram,servicelearningforstudentemployeesthroughthe FederalWorkStudyandCooperativeEducationPrograms,andmentoringtostudents, staff and community organizations.

WithassistanceandguidancefromtheDirectorofResearch,andPlanning,and Institutional Effectiveness,avarietyof researchisconductedinStudentHealthServices,bothforProgramReviewand clinicallybasedpurposesthroughhealthandneedsassessmentsurveys,pointof servicecontactsandsatisfactionsurveys,demographicandutilizationpatternsand program and staff evaluation.

Acentralized,confidentialmedicalmanagementinformationsoftwaresystemisutilized in collecting data and maintaining medical record documentation.Data that may identify specific student information by name is neverused in reporting or research activities.
